Two of eight children records reviewed did not have an emergency contact address provided on the admissions form to review during the time of inspection.

The operational policy handbook did not have a policy for parent rights.

One of Eight children records did not have immunizations in file and available for review during time of inspection.

2 children need a hearing and vision screening for admission file.

Promotion of physical activity needs to be added to the operational policies to include the follow: 1. The duration of physical activity at your operation, both indoor and outdoor. 2. The type of physical activity (structured and unstructured) that children may engage in at your operation. 3. Each setting in which your physical activity program will take place 4. The criteria you will you to determine when extreme conditions pose a significant health risk that prohibits or limits outdoor play. 5. A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.

Procedures for inclusive services to support children with special care needs, needs to be added to the operational policies. The procedures must address the requirements in 746.7222.

An infant was observed sleeping in a crip with a pacifier attached to clothing.

Based on the information received the operation didn't notify the parent that the child had been left outside unattended. The child's grandparent was notified by someone that worked in a building next door that they saw the child unattended on the playground. The grandparent notified the parent of the incident and the parent notified the director

Based on the information received during the investigation the caregiver in charge didn't make sure that all children were accounted for before coming in after outdoor play and after entering the building/classroom.
