Out of the four staff files reviewed, one caregiver was missing training in emergency planning and communicable diseases and two were missing training in managing food allergies, handling hazardous materials, and building and physical premises safety.

Out of four children's files reviewed, one child was missing an affidavit for not getting certain immunizations.

The sheets on some of the cribs in the infant room were not snug, and one set of sheets was coming off of the mattress. This was corrected during the inspection when the sheets were replaced with snug-fitting sheets.

The fire extinguishers were last checked in May 2026. This was corrected during the inspection when the director checked the fire extinguishers.

The infant daily report did not have a section for mood to be recorded.

The director was missing training in communicable diseases and building and physical premises safety. The director took building and physical premises safety training during the inspection.

One caregiver had first aid and CPR training that expired in June 2026.

A fire drill was not conducted in July. This was corrected during the inspection when a fire drill was conducted and the children were able to exit in one minute and fifty seconds.

Two slides were on the grass and did not have any loose-fill around or underneath. This was corrected during the inspection when one slide was removed from the playground and the other was moved to the mulch area so it had the proper amount of loose-fill.

The parent handbook was missing policies for sunscreen, hearing and vision screenings, and some of the parent's rights.

Out of three staff files reviewed, two caregivers were missing several of the required training topics.

The operation made substitutions for breakfast and lunch, but these substitutions were not recorded. This was corrected at inspection when the director recorded and posted the substitutions that were made.

A caregiver used a handwashing sink to prepare food when pouring juice out of a fruit cup. This was corrected at inspection when the caregiver used the trashcan to pour the juice out instead.

There was sufficient evidence found during the investigation to determine that a caregiver used melatonin spray in the classroom before naptime to help the children sleep. This was corrected as the caregiver no longer works at the operation and all melatonin sprays were removed from the classroom.

There was sufficient evidence found during the investigation to determine that a caregiver left a bottle of melatonin spray on the floor within the reach of children. This was corrected when the director removed the bottle from the room when she saw it.

It was determined a child in care was not changed promptly by a caregiver. The caregiver allowed the child to lay down for nap with a wet diaper.

It was determined a child was spanked on the hand by a caregiver when the caregiver determined the child had urinated on themselves.

Observed a toddler use a potty chair and go back to the table to eat without washing hands. This was corrected by the caregiver taking the toddler to the sink and wash hands.

Based on the information gathered during the inspection it was determined that a child was left on the playground unsupervised.
