When arriving at the operation a caregiver was smoking an e cigarette in the parking lot.

During the course of an investigation, it was determined that two caregivers did not demonstrate good judgement when an infant's legs were stuck in the crib rails and they forced the child's legs back into the crib resulting in serious injuries.

During the course of an investigation, it was determined that a caregiver spanked a child.

One caregiver had 6 children in the classroom with the specified age of 12-17 months. This specified age puts the classroom out of ratio by one child. NOTE: this was corrected at inspection.

At the time of inspection one employee was left alone with a group of children, with provisional conditions in place.

The provider failed to notify licensing when a new building / classroom was added and being used by children that was not measured or approved before use by children.

At the time of inspection the caregiver in the three and four year old classroom did not wash a child's after diaper change. This was corrected when the child's hands were washed.

The three step sanitation process was not being used on the changing mat in the 18 to 24 month old classroom.

The livestock panels located on one end of the playground have gaps larger than 3 1/2 inches and smaller than 9 inches.

At the time of inspection the four year old caregiver did not practice good judgment when taking the group of four year old children through the kitchen while lunch was being prepared.

Three shelves in three classrooms over five feet tall were not secured to the wall.

An eight month old was observed sleeping in the crib with a clip attached to the pacifier.
