Operational policies were reviewed and are missing the following information: (18)(F) Outdoor play weather criteria, (23)(a) how parents may access minimum standards online, (23)(c) how parents may access HHS website, (26)(b) Methods for increasing employee and parent awareness of issues regarding child abuse and neglect, (26)(c) Methods for increasing employee and parent awareness ofprevention techniques for child abuse and neglect, (26)(d) Strategies for coordination between the center and appropriate community organizations, (26)(e) Actions that the parent of a child who is a victim of abuse or neglect should take to obtain assistance and intervention, (28) Information on vaccine-preventable diseases for employees and, (30) Procedures for supporting inclusive services to children with special care needs.

Two out three car seats in the transportation van expired 12/2015. Note: this was corrected at inspection when expired seats were removed from transportation vehicle.

There are multiple missing floor tiles in the kitchen and dinning room. There is a hole in the wall by the nursery 1 room. Restroom by nursery 1 has chipping paint. Diaper changing mat in the prek room was torn. Fence surrounding the AC units fell over on one side. Note: diaper changing mat was replaced during inspection.

Based on the information obtained during the course of the investigation, attendance records for two staff members were not recorded / available for review.

Based on the information obtained during the course of the investigation, one caregiver was left in charge of supervising six infants (SAG 0-12mo) during naptime / lunch breaks.

Based on the information obtained during the course of the investigation, a non-walking infant under the age of 12 months did not have an individual crib to sleep in.

Based on the information obtained during the course of the investigation, two staff members are missing 16 hours of pre-service training.

Screen time was provided to a group of children of varying ages including infants in the cafeteria area.

A school-aged child was observed unsupervised in the after school room located at the entrance of the center. The rest of the children and staff were in the kitchen/cafeteria area waiting for breakfast.

Two play yards used by infants have a mattress that was not firm and flat. There are gaps between the mattress and the play yard sides. Note: The caregiver secured the mattress by placing the mattress pad's Velcro tabs through the slits on the play yard.

Written feeding instructions were not available for an infant not ready for table food. The written feeding instructions for a second infant were not current. The date on the feeding instructions form was March 2024.

Daily reports were not available for 4 infants in care.

Three of Five caregiver training records reivewed were missing annual training in Preventing and controlling the spread of communicable diseases, including immunizations.

Two of Five personnel records reviewed were missing documentation showing how the caregivers meet the education qualifications, and a notarized Licensing Affidavit for Applicants for Employment form.

One of Six childrens imunization records reviewed was not current. One of Six childrens records reivewed was missing the health care provider statement.

The operational policies reviewed were missing the following information: (1) Procedures for suspension and expulsion of children; (2) Hearing and vision screening requirements; (3) The Promotion of indoor and outdoor physical activity that is consistent with Subchapter F of this chapter (relating to Developmental Activities and Activity Plan); policies must include: (A) The benefits of physical activity and outdoor play; (B) The duration of physical activity at your operation, both indoor and outdoor; (C) The type of physical activity (structured and unstructured) that children may engage in at your operation; (D) Each setting in which your physical activity program will take place; (E) The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities; and (F) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play. (4) Procedures for parents to participate in the child-care center s operation and activities. (5) Procedures for parents to review a copy of the child-care center's most recent Licensing inspection report and how the parent may access the minimum standards online; (6) Instructions on how a parent may contact the local Licensing office, and access the HHSC website; (7) ) Preventing and responding to abuse and neglect of children, including: (B)Methods for increasing employee and parent awareness of issues regarding child abuse and neglect, including warning signs that a child may be a victim of abuse or neglect and factors indicating a child is at risk for abuse or neglect; (C)Methods for increasing employee and parent awareness of prevention techniques for child abuse and neglect; and (D) Strategies for coordination between the center and appropriate community organizations; (8) Procedures for conducting health checks; (9) Information on vaccine-preventable diseases for employees.
