A caregiver stated that they had ten children in care when they had twelve children in care. This was corrected during the inspection when the caregiver was informed that they had twelve children.

There was sufficient evidence found during the investigation to show that parents were not notified immediately of a situation placing their child at risk.

There was sufficient evidence found during the investigation to show that adequate supervision was not provided when a child was left unattended in a classroom.

One of four classrooms did not have a written activity plan available for review.

Two of the 4 classrooms dd not have a posted activity plan during the inspection.

There were 2 Pre K aged children observed unsupervised in a bathroom during the inspection.

The toy shelves in one Pre K aged classroom were observed to be turned towards the wall making the toys and materials inaccessible to the children.

Garbage was observed to be accessible to children in one classroom during the inspection. A lid was placed on the bin during the inspection.

One bathroom inside of a classroom was observed to be lacking handwashing soap and papertowels.

There was one 21 month old child present in the infant classroom with a 2 month old, resulting in a 19 month age diffrence. The older child was moved to the toddler classroom for the remainder of the day.

The last fire inspection was observed to have been conducted on 9/26/24.

It was observed that an infant's hands were not washed after a diaper change.

Two of four infants enrolled did not have current infant feeding instructions available for review.

It was observed that one caregiver did not wash their hands after changing an infant's diaper.

The handwashing sink in the infant classroom was observed to be used as a rinse or bottle washing sink.

The last documented fire drill was observed to have been complete in March 2024. A fire drill was conducted during the inspection.

Two of four staff files reviewed were lacking the required affidavit for employment.

The grass on the outdoor playground is not being maintained. The grass was observed overgrown and tall in need of mowing.

The most recent inspection was not posted at the operation. The operation still had an old inspection form dated 9/28/23 from previous ownership. This was corrected at inspection when the Director printed and posted the most recent inspection form.

Upon arrival at the operation a caregiver was left alone in a classroom with 16 children who have a specified age group of 3 years old. This was corrected at inspection when a caregiver was added to the room, bringing the room back into ratio.

The written activity plans includes outdoor play however the children have not been participating in outdoor play due to the lack of maintenance of the playground.
