CCR observed a two-month-old infant sleeping in a restrictive device, specifically a "Baby Bouncer." Correction made: The infant caregiver removed the infant from the Baby Bouncer and placed the infant in the crib.

The operation does not have current insurance; its coverage expired on 3/25/2026.

The operation does not have the most recent inspection report posted; the last inspection report posted is dated 10/28/2024. Correction made: The operation replaces the old inspection report with the current inspection report.

During naptime, CCR observed school-age children watching television. Correction made: A caregiver then turned off the television.

The director is missing all required training for the annual training year 4/21/2024-4/21/2025.

An infant/transporter caregiver is missing all required training for the annual training year 9/23/2024-9/23/2025.

During the walk-through, I observed that the infant's room was out of ratio. There were 10 infants in the classroom and the specified age of the children is 11months, and there were only one caregiver present. Correction: Another caregiver came into the classroom.

The toddler playground did not have a second exit on the playground.

The pre-school playground did not have the required six inch surfacing on the playground.

One child did not have immunizations in their file and the other child did not have the current records for a four year old child.

In the infant room, two children were asleep on bare mattress.

Based on the preponderance of the evidence obtained during the investigation conducted by DFPS, a caregiver was yelling and screaming at children.

The restrooms used by the children in care had hand sanitizers in them. There was no hand soap. This was corrected at the inspection when the soap dispensers were placed in the restrooms.

There was no surfacing under the play equipment on the side playground. The turf grass did not have unitary surfacing under it; it felt hard when walked on.

A caregiver did not wash her hands after changing a child's diaper.

A table and flag blocked the exit in the pre-k classroom. This was corrected at the inspection when the table and flag were removed.

A caregiver was changing diapers on a diaper changing table in the pre-k classroom. There was no hand washing sink in this classroom.

In several areas throughout the facility, caregivers had their purses low and accessible to children. NOTE: Corrected during the inspection when the purses were placed out of reach of the children.

There were 11 infants to 2 caregivers in the infant classroom. This was correctged when 2 infants were moved to another classroom.

Three of the six children's records reviewed did not have a health statement from a physician.

The most recent fire inspection report expired on 10/28/21.

Two of the six employee files reviewed did not have the required 24 hours of annual training hours complete.
