A caregiver used an object to hit a child.

The operation did not have the most updated information on how parents can access the Minimum standards online or the HHS web site.

Three caregivers did ot have CPR training.

Three caregivers did not have first aid training.

The play structure's did not have a use zone of six feet, the play structure is only 3 ft and 3 inches from the barrier.

The play strructure on the playground had broken parts.

One caregiver with a hire date of 2/5/24 was missing 20 hours of training for training year 2/5/24 to 2/5/25. One caregiver with a hire date of 7/24/23 was missing 8 hours of training for training year 7/24/23 to 7/24/24. One caregiver with a hire date of 5/1/24 is missing 3 hours of training for training year 5/1/24 to 5/1/25. Corrected: When using 20 hours of training for training year 2/5/25 to2/5/25, using 8 hours for training year 7/24/25 to 7/24/26 and using 3 hours for training year 5/1/25 to 5/1/26.

The operational polices did not have information on Parent Rights.

The operation failed to make a report when a child was placed at risk.

The caregiver did not intervene when a child was seen choking from a string wrapped around their neck.

The operation did not provide an incident report when a parent made an allegation that a child was place at risk.

A caregiver was using a cell phone while tasked with supervision of children.

During the inspection it was observed that parents were not being allowed access into the main part of the operation while picking up their children. They were stopped in the front office and their children were brought out to them.

During the inspection it was observed that the toddler sink was not equipped with soap and the school age toilet was not equipped with toilet paper.

During the inspection it was observed that 4 infants were having screen time, using their caregiver's tablet, sitting in highchairs.

During the inspection it was observed that a caregiver removed their gloves after changing a diaper and did not wash their hands before moving on to the next child.

During the inspection it was observed that the changing pad was not sanitzed between each diaper change.

During the investigation inspection it was found that the operation was not able to show records that they have been conducting their daily or monthly inspections to ensure the safety of the building and grounds for the children present.

During the inspection it was observed that a child's diaper was changed and the caregiver did not wash their hands afterwards.

During the investigation it was found that the operation had been deficient in supervision when caregivers were unaware of some of the school age children's behaviors and actions.

During the investigation it was found that there was a 2-3 day period in February 2025 in which the operation did not have their air conditioner running and this caused the operation to be too hot inside for ideal and/or safe conditions for the children present.

During the investigation it was found that the operation is sometimes out of compliance with state required caregiver to child ratios. One caregiver will be left responsible for a higher number of children than they should have by themselves. This has occurred with the infants, toddlers, and school age children at different times in the past month.

During the inspection dead roaches were observed in the building and garbage was observed on the playground.

During the investigation inspection it was found that the operation had a broken toilet lid, with broken porcelain, accessible to the children in care. It was also found that the changing table in the infant room has a broken strap and is unstable, causing the potential of a baby sliding off. In it's current state, it is not suitable for use and presents a safety hazard.

During the inspection it was observed that the infants had one caregiver with seven infants present. This classroom was over ratio by two infants. It was also observed that there was one caregiver with 12, two year olds. This classroom was over ratio by one toddler.

During the follow-up inspection an infant was observed sleeping in a bouncer seat.

During the follow-up inspection it was observed that a classroom with children under the age of two were having screen time on the caregiver's cell phone.

During the inspection it was observed that the toilet in the toddler classroom was not equipped any toilet paper as required for independent use.

During the inspection it was observed that a classroom with children under the age of two were watching a children's show on the classroom's tablet.

During the inspection it was observed that a caregiver changed a child's diaper and then sat them at the table and served them a cup of milk without washing their hands or the child's hands first.

There are photographs that show pests in the director's office.

There was no proof that there was a written agreement between the parent and operation that food would be brought from home.

The first aid kit was incomplete on the transportation van. The kit was missing tweezers and a thermometer. The first aid kit in the building was missing tape. Note:corrected at inspection.
