1 staff member is missing the certificate for transportation.

The transportation van is missing a child safety alarm.

Is missing documentation at the child-care center that shows when your center first purchased or leased a vehicle.

The director is missing annual training in hazardous materials and handling emergencies due to food or an allergic reaction.

Multiple tv's were on playing various children's programs on YouTube while children were doing activity centers. Note: this was corrected at inspection when the director turned off the tv.

The center's operational policy for Emergency Preparedness Plan is incomplete and needs to include information about how we transport infants.

The center's operational policies is incomplete for promotion of physical activity.

Children receiving transportation services from the operation are being dropped off at the elementary school in the morning before 7:08am when the school allows staff to open the doors to supervise the children. Prior to 7:08am, the school does not provide supervision, nor do they assume responsibility for the well-being of the children.

There is no documented proof of training for the contracted transportation service by the operation's owner/director.

4 children were left unsupervised while a caregiver took a child to the restroom down the hall way. Another child walked out of the classroom to the restoom alone during the follow up inspection.

Only the director was observed with 5 children during naptime. An additional staff person was not present in the building during nap time. Note: The director called a caregiver in who arrived during the inspection.

The director did not practice good judgment and was observed driving the daycare van into a parking spot. Upon entering the daycare center, 5 children were observed inside the daycare center alone with no supervision.

The director did not ensure compliance was maintained with the minimum standards as children were left unsupervised without an additional caregiver present. Note: An additional caregiver arrived during the inspection

A child was observed sleeping in the crib with a bottle.

1 out of 5 children's files evaluated did not have a food allergy plan available for review.

2 infants were observed sleeping in swings. Note: This was corrected as the caregiver removed both infants from the device, placing them into a designated crib.

2 infants were left unsupervised in the infant classroom during the inspection.

The operation did not have in their emergency preparedness plan on how they would relocating children who are under 24 months of age, who have limited mobility or assistance. Corrected: The director updated the emergency preparedness plan.

The director did not have updated transportation training that expired 11/5/2022.

The operation did not have documentation of their most recent fire inspection.

The operation did not have any loose-fill surfacing material under the moving equipment.

In the back bathroom the diaper changing mat had several tares in the middle of the changing mat.

There is a child in care that has a listed food allergy but there is not a list posted with the child's name and food allergy.

One child in care does not have a completed and signed food allergy plan in their file.
