There was no current annual Health Inspection Report available for review during the inspection. Health Inspection was observed dated 6/12/25.

Four caregivers were observed caring for children during the inspection without eligilble background checks. One caregiver with pending background was observed leaving the operation during the inspection.

Infant feeding instruction sheets were observed not updated every 30 days for infants not able to eat table food.

Bottles were observed not labeled in the infant room. This was corrected during the inspection when the caregiver was observed properly labeling the bottles.

There was no statement of health for two guinea pigs observed in a classroom to review during the inspection. This was corrected during the inspection when the guinea pigs were observed being removed from the operation.

A crib mattress was observed torn and unwashable in the infant room. This was corrected during the inspection when the caregiver was observed replacing the crib mattresss with a washable mattress.

Diaper changing supplies were observed in reach of children on the changing table during the inspection. This was corrected during the inspection when the caregiver was observed removing the supplies out of the reach of children.

One caregiver was observed caring for 5 infants ages 6-7 months old. This was corrected during inspection when one infant was observed being removed from the classroom and placed in another infant room with two caregivers.

There was no infant feeding instruction sheet available for review for a 3-month old infant during the inspection.

Clorox wipes, bleach sanitizing solution, and hand sanitizer was observed stored on the diaper changing tables in infant and toddler room accessible to children during the inspection. This was corrected during the inspection when caregivers were observed removing the hazardous materials and storing them out of the reach of children.

There were no activity plans posted in the infant rooms or one toddler room during the inspection.

Multiple Sheets in the infant room were observed not snugly fitted in the cribs.

Sippy cups in the toddler room was observed not properly labeled with the child's first name and first initial of last name. This was corrected during the inspec tion when the cups were observed being properly labeled.

A water drainage hole was observed open and uncovered accessible to children on the playground.

It was observed that two 12 month old infants in a walker.

A caregivers was observed providing direct care to children in the two year old classroom prior to receiving eligibility status.

An infant was observed sitting in a restrictive device for 30 minutes, while crying.

Three caregivers were observed providing direct care to children in the classroom prior to receiving eligibility status.

It was observed an infant crying for 30 minutes and the caregiver did not attend to the infant's needs, the caregiver sat on the floor with the other infants and had her back turned to the infant crying.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

Of the 10 children's records evaluated, 2 lacked their health statement.

Operation did not have a current fire inspection.

Based on the information obtained during the DFPS investigation, there is a finding of a child being inappropriately disciplined by a caregiver while in care at the operation.
