The operation does not have a current sanitiation inspection as required.

During the inspection I observed an changing mat with a rip.

During the inspection I observed a caregiver leave 5 infants unsupervised in a classroom when she stepped out to place a carseat in a area outside of the classroom.

A caregiver was observed not sanitizing a diaper changing area after diapering a child.

During the inspection I observed all of the cribs in the infant room placed close together where infants could reach into another crib. This was corrected when the director rearranged the cribs.

The school-age classroom has a wooden board with exposed nails leaning against the wall. The operation back door leading to the playground walk-through has broken tiles, exposed nails in the wood, and a trashcan accessible to children.

Toddler playground loose fill is less than an inch ruler.

I observed 5 children under 8, transported in a bus van without booster seats.

The infant room was not in ratio. Specified age 0 - 11 months. There were 5 infants and one caregiver. Note: Another caregiver did come into the infant room after inspection.

The use zone to enter the slides is not six feet from the perimeter.

Garbage trash was accessible to children.

During the walkthrough of the after-school room, I observed a Lysol and several other bottles of cleaning products in an unlocked sink cabinet accessible to children.

I observed a stuffed animal toy hooked onto an infant pacifier while lying in the crib.

A newly purchased vehicle for Transportation at the center does not have an electronic safety alarm installed which is required.

1 of 12 staff members had first aid/cpr that expired.

1 of 6 staff files evaluated did not have the emergency care authorization filled out and signed. 2 of 6 files did not have the date of admission in the file. This was corrected during the inspection. 1 of 6 files did not have the complete address for the emergency contact and 1 of 6 files did not have the name and address of the child's school.

Eight employee files were reviewed. FIve of the files did not have a documented high school diploma or its equivalent

Ten children's files were reviewed. Eight of the files have not been updated in over one year.

Ten children's files were reviewed. One of the files did not have an admission date. Two of the files did not have health providers information listed. One file did not have signed acknowledgment of having received the operational policies. One file did not have the emergency authorization for care completed. One of the files did not have a copy of the most recent immunizations on file.
