The emergency plan did not include a reunification plan for parents and children in an emergency for either an evacuation or lock down.

The emergency preparedness plan did not include the continuation of care procedures.

The emergency preparedness plan did not include how children 24 months and younger and/or with mobility restrictions will be evacuated. The plan also did not discuss the orderly movement of children and staff regarding a shelter in-place or lock down.

One child's record did not contain a health statement after being enrolled for over 12 months.

18 children admission information did not have the swim/water activity permission and required information.

6 personnel records did not contain Form 2912 Pre-Employment Affidavit for new hires hired after 9/2023.

Two school age children did not have the required school information in their files.

Training records were not made available to review and evaluate during the inspection.

The list of employees leaving the center in 2024 was not submitted by the deadline.

The background check list had not been updated within the last 3 months.

I observed a sink with cleaning supplies stored underneath with no child safety lock. The items were marked "Keep out ot the reach of children". This was corrected at the time of the inspection when all the items were removed from the cabinet and placed in an inaccessible area.

During the walk through of the facility the infant room was observed without adequate lighten. CCR and Caregiver was unable to determine the number of children in the classroom.

9 children were observed in a classroom without caregiver taking a nap ages 2-3yrs old. A 14months child was observed left in a pack and play in a classroom without supervision. 9 children were observed in the cafeteria without proper supervision. The caregiver was in the kitchen preparing snack.

The background checks had not been validated in the last 3 months.

The center did not report the number of employees leaving employment the previous year.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

Of the nine staff records evaluated, all lacked training in the following health and safety topics for their respective training years: 1. Emergency Preparedness 2. Communicable diseases and immunizations 3. Administering medications 4. Preventing, controlling and responding to emergencies related to food allergies 5. Understanding Building and physical premises safety 6. Handling, storing and disposing of hazardous materials The caregivers will need to receive make-up training hours. The make-up training hours will not count towards the current training year.

Based on the number of deficiencies cited, the director is not operating the center in compliance with Minimum Standards, rules and laws.

A caregiver did not know how many children she was responsible for, Caregiver must know the number of children in their care at all times.

Seven staff did not complete an orientation that covered the following topics: 1. Administering medications 2. Preventing and responding to emergencies related to food allergies 3. Building and physical premises safety 4. Handling and storing of hazardous materials 5. Precautions in transporting children

Of the 10 children records evalauted, four lacked the address for the emergency contact.

Children were observed watching television during snack time. This was corrected at inspection.

Toddler age children are only going outside twice a day. Toddler wage children must be given the opportunity for outside play twice a day, weather permitting.

A caregiver was observed washing the hands of 2 year old with a wet papertoiwel. Children of this age must wash their hands with running water and soap.

The written activity plan does not include the opportunity for outside play twice a day. CHildren mist be given two opportunities for outside play each day, weather permitting.

A daily report sheet is not being provided to parents of all infants in care. An infant is defined as a child age 0-17 months. Parents of all infants must be provided a daily report sheet.

The following supervision issues were observed: 1. A group of school age children were left unsupervised while the caregiver went to the front door to retrieve children that been dropped off by the van driver. The children exited the van and entered the building unsupervised. 2. A child was left unsupervised in the cafeteria while another group of children was being retrieved. 3. Children were allowed to enter the building and go to the playground unsupervised.

The operational policies is lacking the following information: 1. Safe sleep policy to infants 0-12 months old. 2. Promotion of physical activity and outside play 3. The contact information for the local Licensing office is incorrect 4. Inform parents that a comfortable place for a mother or breastfeed is available and breast milk can be provided. 5. Inform parents that the center is required to receive annual training regarding child abuse and neglect.

Of the 10 children records evaluated, 8 lacked the address for the children's doctor.

Of the 10 children's records evaluated, two lacked the name and telephone number of the school the child attends.

Caregivers are washing the hands of infants who are old enough to reach for running water with a soapy papertowl. Once the child is old enough to reach for running water, the child must wash their hands with running water and soap
