A signature is needed on the food allergy emergency plan for one child enrolled.

The operation utilizes a numbered cot/chart system for nap. During today's inspection I observed children asleep on numbered cots however the cot did not match the posted chart. A number/child assignment map was not available in the Pre-K and Fours classroom.

The PM snack menu showed children would be served fresh apples and sun butter. During today's inspection children were served oranges and crackers as well as oranges and pancakes for PM snack.

There was no sufficient walk and work space between cots in the Infant B classroom.

During nap time, children were observed sleeping behind a shelving unit. The shelving unit obstructed the caregiver's view, preventing the caregiver from maintaining visual supervision of the children.

Updated feeding instructions are needed for eight children.

I observed unlabeled cribs used for children enrolled.

A caregiver did not wash their hands after diapering a child. The caregiver was beginning to diaper another child. Note: This was corrected during inspection. The caregiver washed their hands before diapering the next child.

Upon entering an infant classroom, one caregiver was observed with ten children ages 14-19 months old. The specified age group for the class was 12-17 months which allows one caregiver to supervise a maximum of 5 children. The caregiver was over ratio by 5 children. A caregiver in another classroom was observed with 9 children ages 13-20 months old. The specified age group for the class was 12-17 months which allows one caregiver to supervise a maximum of 5 children. The caregiver was over ratio by 4 children.

A child did not wash their hands after having their diaper changed. Note: This was corrected during inspection. The child washed their hands before returning to play.

During today's inspection I observed stored medications that were expired. Note: This was corrected during inspection.

Health statements are needed for two children enrolled.

Equivalency documentation is needed for three caregivers.

The last fire inspection was completed 08/26/2024.

4 caregivers files evaluated did not contain documenation showing training was completed in Early Childhood Brain Development.

2 out of 4 staff files evaluated did not contain documentaiton of CPR and First Aid training.

Documentation showing the smoke and carbon monoxide detectors were tested in June and July of 2024.

A caregiver was obsserved with 8 children in the Toddler C classroom. The child to caregiver ratio should've been 5:1. The caregiver was over in ratio by 3 children.

A caregiver was observed in the Toddler classroom with direct access to children. The caregivers background check has been inactive and the operation has not received an eligibility notification from CBCU stating the caregiver can be present. Note: The caregiver left the operation during the inspection.

1 out of 4 children's files evaluated did not contain a completed health statement.

A yellow bucket of dirty mop water was observed in the toddler room during the inspection.

The nap cots children were resting did not provide sufficient walking space during the inspection. Note: The operation rearranged the cots during the inspection to allow for sufficient walking space.

The annual sanitation inspection has not been conducted since May 9th, 2023.

An aerosol spray can of Goo Off Graffiti was used in a classroom causing a strong chemical odor in a room where children are present.

A TV was observed on top of a shelf leaning on the wall and was not mounted or anchored. Note: This was corrected at the inspection when the caregiver removed the TV from the room.

The operation did not provide CCR access to an incident report upon request.

A caregiver was observed in a classroom with direct access to children. There is no active background check for the caregiver and the operation has not received an eligibility notification from CBCU. Note: The caregiver left the operation during the inspection.

A caregiver did not practice good judgment when sending a child home with the incorrect bottle of milk.

1 out of 10 staff files evaluated did not contain a high school diploma.

The first aid kit evaluated during the inspection did not contain a pair of tweezers.

2 out of 10 children's files evaluated did not contain a health statement.

Training certificates were not available for review upon request for 10 staff files evaluated.

1 out of 10 staff files evaulated did not contain a notarized affidavit for employment.

There are seven infants that did not have an updated infant feeding instruction.

Cubbies in the Prek 4 classroom did not have labels to specify which cubbie belonged to each child. Children's blankets were placed together in one large cubbie or storage bin.

Mats in the classrooms are labeled with numbers but there are no sleep charts to specify which mat belongs to which child.

There are four infants that did not have an updated infant feeding instruction. They were last signed in July and August 2022.

I observe caregiver present that was missing current background check.

Based on the information obtained during investigation, it was determined staff did not supervise children when a child was left unsupervised in the classroom.
