Infant room matress was observed to be torn and not maintained as a waterproff surface.

There were three out of ten children records observed with no food allergy emergency plan available for review.

Mold was observed under the cabinet in the infant classroom This was corrected during the inspection when I observed the owner and staff member remove the mold and sanitize the area.

School age children were watching a movie during nap time.

A child was grabbed by the arm by a caregiver during the inspection when moving them to another classroom.

Caregivers were heard yelling at children during naptime to lay down and get on their nap mats.

An incident report was not completed for a child who was scratched while in care.

A caregiver with a provisional background check was observed alone with 5 children in the infant room. Note: Another caregiver came into the room during the inspection.

An infants was observed sitting in a high chair and the safety straps were not fastened. Note: The provider corrected this during the inspection by securing the safety straps.

Infants were engaged in screen time during the follow up inspection. Note: The provider turned the screen off during the inspection.

A caregiver stated they had 19 children in the classroom when there were only 18 children accounted for.

An infant was observed sleeping with a pacifier that had an attachment during the inspection.

Based on the information obtained, there was sufficient evidence to show children are not given snacks due to their behavior in the classroom.

A total of 24 children were observed in the school-age classroom, 5-11 years of age. A total of 6 children were 5 years of age, and 5 children present were 9 years of age. A total of 4 children were 6 years of age. A total of 3 children were 7 years of age. A total of 4 children were 8 years of age. One child was 10 years of age, and one child 11 years of age.

During the inspection, the inspector noticed a fire extinguisher in one of the vans used to transport the children but it was discharged. Corrected at inspection when the director placed another fire extinguisher on the vehicle.

The inspector observed a tear in the diaper changing mat that was being used in the infant classroom. This would prevent it from being non-absorbent. Picture was taken.

The emergency preparedness plan does not address how children will be accounted for during an evacuation.

There were multiple dead roaches in the toddler bathroom that had not been disposed of. Note: The provider disposed of the dead roaches during the inspection.

An infant was sleeping an a swing with the safety straps secured in the infant room during the inspection. Note: Discussed the risk of infants sleeping in restrictive devices and the caregivers moved the infant to their crib.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022 revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022 as required. NOTE: The operation reported the number of employees who left in 2021 on 3/1/2022 after the deadline.
