In the infant room, there is an exposed hole in the wall with dust and insulation visible where the air conditioning unit is located.

During the inspection, the operation did not have a current copy of the fire inspection report available for review.

During the walkthrough, I observed several cleaning products in an unlocked sink cabinet that is accessible to children. Correction made: The infant caregiver removed the cleaning products and stored them in a location that is not accessible to the children.

The operation does not have an updated fire inspection; the last fire inspection was on 08/28/2024.

Three employees' hire dates were missing from the file. Correction: The director documented the three employees' hire dates.

A caregiver is missing 24 clock hours of required annual training for December 2023 to December 2024. The primary caregiver needs to complete 24 clock hours of make-up training that will not count towards the current training year.

The first aid kit contains ointments and antiseptic solution that have expired.

During the inspection I observed a caregiver not wash her hands after feeding one infant and beginnning to feed another infant.

During the inspection 2 out of 3 caregivers in a classroom was found to not have had current training in SIDS, shaken baby syndrom, head trauma and early brain development as required.

A caregiver redirected children in care by tapping them on the hand and using a toy to tap on the child?s head.

The director did not self-report an allegation brought to their attention by another caregiver.

During review of children's records 1 out of 6 records evaluated did not include physician information.

During review of children's records 6 out of 6 records evaluated did not have a health statement.

During revierw of records 1 out of 6 files reviewed did not include signed policies.

During review of records 1 out of 6 records evaluated did not include a notarized affidavit.

During review of children's records 6 out of 6 records evaluated did not include all required information for emergency contacts.

During review of children's records 1 out of 6 records evaluated did not include emergency medical authorization.

During review of personnel records the director was found to not have completed the 30 hours of annual training as required.

During review of infants 3 infants were found to not have current feeding instructions.

During the walkthrough I observed two changing mats with holes.

During the inspection I observed 1 caregiver caring for 5 infants with a SAG of 0-11 months when the second caregiver walked out of the classroom to go into another room. This was corrected when the caregiver came back into the classroom.

A caregiver was observed supervising 4 infants in the infant classroom without cleared eligible background check results.

During the follow up inspection it was found that one caregiver was present counted in ratio with a inactive background check.

During investigation inspection it was observed that the 12-18-month room only had one caregiver with 11 children during naptime.
