One out of six caregivers did not have an affidavit available for review

A fourteen-month old infant was grouped with a forty month old child.

Director is missing twenty-two hours of annual training.

A caregiver did not know the ages of the children and the information was not posted.

The play house on the playground structure is broken and has a nail sticking out of the wood.

The director is struggling to maintain compliance and ensuring that the operation is ran in a safe manner. The director is often away from the operation and is unaware of the concerns observed. Children are seen opening doors and running out of classrooms and the caregivers are having a hard time with classroom management. The operation is starting to have repeated citations and I have discussed my concerns with the director multiple times and no progress has been made.

An operation violated a condition when a caregiver with a provisional was alone in the infant classroom during nap time. This was corrected during the inspection when another caregiver returned from lunch break and went into the classroom.

One caregiver with a condition was observed supervising two infant classrooms by themselves.

Three live ant beds were observed on the playground structure.

While I was driving on the street getting ready to turn into the daycare parking lot I observed a caregiver grab a child by one arm, completely lifting the child off the ground and proceeded to walk the child to another caregiver who was by the side door and that caregiver also lifted that child up by one arm and took them inside. The teacher was by themselves outside on the playground and was transitioning the children from outside.

During the walk through I observed one caregiver walking between both infant classrooms supervising the children. There was three infants in one classroom and one six month old in the other infant classroom. The same caregiver also walked completely out of the infant room to let in a parent and caregiver.

During the follow-up inspection I asked the two caregivers present who was in charge when the director is not present and both caregivers did not know that someone has to be in charge when the director is not present. Both caregivers stated I get here when there's already children, how can i be in charge.

During the follow-up inspection, I observed two children ages nine and six siting in the front toddler classroom by themselves. The two children did have a tablet and when I asked them were there teacher was the nine year old stated my mom works here and thats why I am here. When I approached the parent who is also the cook she stated the two children should have been sitting in the classroom with the school age teacher. I also observed an eleven year old child sitting in a dark office laying on a couch. When I asked the child what was wrong, he stated he was not feeling good. There was no teacher supervising the child. At 3:26 pm a caregiver brought the child out of the office and took them to the classroom.

While waiting for someone to answer the front door I observed the owner/director running passed the front door and back into the hallway were the classrooms were. I thought the director was coming to open the door for me but instead ran into a hallway. When another caregiver finally opened the door I conducted my walk through and asked the director why she ran passed the door. She initial stated that was not her and then said she went to the front desk to get something and immediately came back to the children. During the time that she ran to her desk, no other caregiver stepped in to be with the children.

A child under the age of twenty-four months was observed watching coco melon. This was corrected during the inspection when I observed the director turn the television off.

Two out of five children were missing a date of admission. This was corrected during inspection when the director added the admission dates on the form.

One out of three staff members did not have a Pediatric CPR and Fist Aid certificate available for review.

Toddler caregiver did not know the age of the children and the information was not posted showing each child's name and age. This was corrected during inspection when the director gave the caregiver the children's information and posted it on the wall.

The sanitation inspection was not available for review.

The small playground had multiple wooden boards that is accessible to children and the main playground area had a broken fence.

The transportation vehicle does not have the child-care center's name or telephone number on the outside of the vehicle.

The transportation vehicle did not have an electric saftey alarm.
