The director's CPR-first aid training expired August 2025.

The director is missing annual training must be relevant to the age of the children for whom the operation provides care, for the subjects of: Preventing and responding to emergencies due to food or an allergic reaction; Understanding building and physical premises, 2 hours in child growth and development.

The operation last validated background checks on November 20, 2025. Note: this was corrected at inspection when the permit holder validated.

The operation failed to submit a request for an initial background check for each person required to have one. 1 caregiver and 1 contracted driver both in ratio and without initial background checks.

The current fire inspection is expired and needs updating.

One volunteer ended their role with the operation on 5/8/25. Note: this was brought into compliance when the provider inactivated the background check.

Documentation of drills was not provided to CCR for review.

The director needs to complete 2-hours of transportation training as the operation offers that service.

Two active ant beds and two metal posts are present on the playground. The metal posts were once supporting a tree and no longer being used for that purpose.

1 out of 4 children's records missing the address of another responsible individual (friend or relative) who should be contacted in an emergency when the parent cannot be reached.

The operation offers dinner service to after schoolers with no posted menu. This was corrected at inspection when the director created a daily menu for the parent board.

A tv that was sitting on a child size table in the main play space was not anchored to the table or the wall. Note: this was corrected at inspection when the director removed it and placed in the director office.

The playground had a sharp and jagged piece of tile in the grass. Note: this was corrected at inspection when the director had the trash removed from the playground.

The transportation vehicle is missing the first aid kit.

The first aid kit is missing emergency guide, medical tape, thermometer and multiple size band-aids.

The operation's name and telephone number must be clearly visible on the outside of the vehicle or kept inside the vehicle.

The operation did not have the sign-in/sign-out log sheet for 10/28/24, 10/23-10/25/24, and 10/18/24.

The operation transports multiple children from two schools to daycare with no lists of children.

The operation transports children without keeping a copy of the Emergency medical transport and treatment authorization forms for each child being transported.

The transportation vehicle is missing the fire extinguisher.

At the time of inspection, the full background check has not been completed.

The operation needs a Sanitation Inspection.

The kit is missing: guide, scissors, tweezers, thermometer, and multiple size bandages.

Hornet nests are present in the active play space of the play ground.

The operation needs an initial fire inspection.
