The caregiver did not exemplify good judgement when approaching the child and failed to engage in deescalation tactics aligned with the operation's policy, which resulted in a physical altercation between her and the child.

An incident took place at the operation that placed a child at risk of harm or injury and was not reported to licensing within the required 48-hour time frame.

During the walkthrough three caregivers provided the incorrect amount of children they were resposonsible for. This was corrected when caregiver recounted the children.

A child younger than 12 months was observed in a crib with a pacifier with a beaded string pacifier clip clipped to their shirt while asleep in the crib. This was corrected when the caregiver removed the beaded pacifier string clip from the child's shirt.

Based upon the information gathered during the investigation, evidence was available to support the safety precautions for unloading children was not followed. Two staff conducting transportation services did not account for all children present on the vehicle. As a result, a child was left in the van unattended for approximately 30-40 minutes.

Based on the information provided, bus logs were not documented for children transported from September 1- September 28, 2025.

Based upon the information gathered during the investigation, sufficient evidence was available to support neglectful supervision due to a child being left in the vehicle unattended.

Based on the information obtained, a parent was not notified of an incident that placed their child at risk. A child was left unsupervised on the transportation vehicle at 3:58PM and was not retrieved until 4:33PM. Based on the information obtained, the parent was not notified until pick-up at 5:45PM. The operation failed to notify the parents immediately of a serious incident directly involving their child.

Based upon the information obtained during the investigation, sufficient evidence was available to support, the driver failed to follow the facilities procedures regarding safety precautions for loading and unloading children which resulted in a child being left in the center's vehicle unattended.

The operation did not have the current fire inspection.

Caregiver changed diaper and did not wash the child's hands. This was corrected during inspection. Later, compliant hand washing procedures were observed,

It was observed that the fitted sheets on infant mattresses were not the correct size for the mattresses being used. this was corrected at inspection when the appropriately sized sheets were applies to the infant mattresses

During the inspection it was found that the operation did not conduct a fire drill in the month of December. Their last fire drill took place 11/27/2024 and is required monthly.

During the inspection it was found that the fire extinguishers in the transportation vans had not been serviced since Nov 2023. These extinguishers are required to be serviced annually.

During the inspection it was observed that at least one of the classrooms was served breakfast, missing 1/3 required components. This is corrected when milk was served with the waffles and strawberries.

During the inspection a child was observed being served breakfast without having washed their hands first.

The fenced outdoor playground did not have the required 2 exits. There was only one exit/entrance.
