One caregiver was missing the following training: (1)Recognizing and preventing shaken baby syndrome and abusive head trauma (2)Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS)

During today's visit I observed two non walking eleven month old infants on mats.

Snug fitting sheets are needed in cribs in one infant classroom.

A caregiver was observed with a group of 8 children ages 4-12 months old. The caregiver did not have an eligible background check. The caregiver was removed from the classroom during this inspection.

Based on the information obtained during this investigation, there is sufficient evidence to support a firearm was brought into the operation.

Based on the information obtained during this investigation, it was determined the director did not demonstrate good judgement when bringing a firearm into the building.

Medication stored in its original container is needed for one child in care.

The caregiver stated twelve children were present, however thirteen were counted in the room. A caregiver responsible for an outdoor group of children ages three-four years old was unaware of the number of children responsible for. The caregiver stated eight children, however nine were present.
