Holy Spirit Early Childhood and Primary School

The outdoor play area is in need of loose-fill surfacing.

The operations last sanitation inspection was completed on 09/26/2024.

During the course of this investigation, it was determined that the operation failed to notify Child Care Regulation within the required timeframe that a child in care received medical evaluation by a healthcare professional following an injury.

The use zone to the sides of a slide do not extend to the required 6 feet. An additional 2 feet is needed on one side. Note: The provider removed the slide during the inspection.

The annual sanitation inspection has not been conducted since August 23rd, 2023.

During the inspection a caregiver was observed changing diapers and did not have children was hands afterwards. Note: This was corrected at inspection when caregiver was instructed to wash each child's hand after having their diapers changed before they go back to play.

A small slide was observed installed over grass without the required loose-fill material or unitary surfacing. Note: The play structure was removed during the inspection.

Education written in a foreign language was not translated to English for one out of four staff files evaluated.

1 caregiver has not completed the skill-based portion for pediatric CPR training.

3 out of 10 staff files evaluated did not contain the original certificate of completion or a copy for CPR and First Aid training available for review during the inspection.

4 out of 10 staff files evaluated did not contain proof of meeting education requirements.

Cribs in the infant room were not labeled for each infant.

The annual sanitation has not been conducted since August 9th, 2022.

Documentation of current liability insurance for the operation was not available for review during the inspection.

Documentation showing the director has completed the full 30 hours of annual training was not available for review during the inspection. An additional 20 hours is needed.

3 out of 10 staff files evaluated did not contain the full 24 hours of annual training.

During the inspection, a caregiver was observed with direct access to children in care. The operation has not obtained an eligiblity notification from CBCU stating the individual can be present. Note: The caregiver left the operation during the inspection.

During the inspection infants were observed in their cribs with pacifiers that contained an attachment piece. Note: It was discussed to only use pacifiers that do not contain attachments as they pose a risk of strangulation and suffocation.

A child had not been signed in for the day. Note: The caregivers noted the child's time of arrival on the attendance log in the classroom during the inspection.

During the inspection, a caregiver was observed in a classroom and the operation did not have an eligibility notification from CBCU stating the caregiver could be present.

Based on the information, the operation did not report a situation that put a child at risk to CCR within the required timeframe.

Based upon the information obtained, there is sufficient evidence to support that a caregiver was on their phone while caring for children.

Based upon the information obtained, a caregiver did not count the number of children before going outside and left a child alone in a classroom for approximately 2 minutes.

Based on the information obtained, the Licensing Incident Report was not shared with parents regarding a child that was left alone in a classroom.

Five classrooms were observed with children napping in the exit walkway. This was corrected at inspection when each caregiver of the classes moved the children to another location.

Four caregivers was observed at the operation without notification of eligibility from the background check unit.

One caregiver was observed present at the operation in the 3 year old classroom with an inactive background check.
