A nail was sticking out of the wooden barrier on the playground

An individual with a pending background check was observed caring for children in the preschool classroom. This was corrected during the inspection when the individual was observed leaving the operation

While interviewing a caregiver they admitted to smoking a vaporizer in front of children while they were outside on the playground. This was corrected when the director informed me that the caregiver was written up and issued a warning.

There is sufficient evidence to support that inappropriate discipline is being used when a teacher admitted using corporal punishment on their child while the child is in daycare

Infants were observed watching TV on a tablet during nap time. This was corrected during the inspection when the director removed the tablet

A 14 month old infant was observed in the toddler classroom and the oldest child was 36 months making it a 18 month difference in age.

A 14 month old infant was mixed with a toddler classroom during nap time. There were a total of 13 children and the specified age group was two. Due to the child being an infant the caregivers ratio could not be reduced leaving the teacher out of ratio.

Mushrooms were observed on the playground.

The last fire inspection report was dated 02/20/2025 and the fire marshall has not come out yet to conduct the inspection for 2026

The playground's mulch is below the specified amount and not being maintained.

Multiple live ants beds were observed on the playground

Three mushrooms were observed on one of the swing sets on the playground. This was corrected during the inspection when I observed the director remove the mushrooms and place them in the trashcan.

The fire inspection report was not available for review.

Based on the information obtained during the investigation conducted by DFPS it was determined that there were five infants being supervised by one caregiver. Based on the information obtained the caregiver was supervising two four-month-olds, one three-month-old and one fourteen-month-old. The other infants age was undetermined however according to minimum standards the specified age group would be four months.

Based on the information obtained during the investigation conducted by DFPS the infant caregiver admitted to allowing an infant to sleep in a bouncer and denied placing the infant in the crib.

Multiple bolts were seen sticking out of the black barrier around the playground structure.

The carbon monoxide documentation was not available for review. This was corrected during the inspection when the director tested the alarm and documented the test on the form.

The dining room and hallway ceilings were cracked leaving the wood under the ceiling exposed.

The fire inspection report was not available for review.

The operation uses the procare app for transportation and does not have an emergency medical form for each child being transported on the vehicle.

An Employee was seen present with out an active background check. This was corrected at inspection when the director sent the caregiver home.

1 staff file is still missing the notorized licensing affidavit.

There is still broken play equipment in the outdoor play area.

1 staff record is still missing education documentation.

The feeding sheets for 4 infants have not been updated within the last 30 days.

During the inspection infants were sleeping with pacifiers in their mouths that contained an attachment to their clothing. Note: It was discussed that pacifieres can be used during sleep but must no contain the string or any other attachment to the clothing as it poses a strangulation hazard.

One of the nap cots were not labeled during the inspection.

During the inspection and 8 month old infant was placed in their crib which contained blankets and toys. Note: The provider removed the items from the crib during the inspection.

1 out of 10 children's files evaluated were missing current immunization records.

The nap cots did not contain adequate walking space between each cot.

3 out of 7 staff files reviewed did not contain a current certificate of CPR and First Aid training completion.

During the inspection an infant was sleeping in a bouncer equipped with safety straps. Note: The provider was instructed to remove the infant and put them in their individual crib.

There is broken play equipment in the outdoor play area. The electrical outlets in the after schooler room is missing the outer panel and the inside of the wall is exposed.

One bottle in the infant room was not labeled.

During the inspection a television was not mounted or anchored in the classroom.

The training certificates for the director were missing for 4 hours of the required annual training.

Annual fire inspection was not current during inspection. The inspection on file expired since 1/12/2022.

Based on the information obtained duirng investigation incident was not shared with parent during pick up.

One out of four staff records checked did not have current first aid cpr training.

One caregiver with no previous experience with child care did not have the required 24 hours of preservice training. Caregiver only had 8 out of the 24 hours of preservice training.

Logs around the playground swing were loose and there were nails sticking out. Pictures were taken.

One infant bottle was not labeled. This was corrected during inspection.

Fire extinguisher in the vehicle used to transport children have fire extinguisher that was not mounted.
