The children were observed without safety straps on while sitting down for lunch. Note: this was corrected at inspection.

The director and caregiver/driver did not have the required transportation training available for review during the inspection.

The playground equipment surface area did not have six inches of loose fill during the inspection.

The fire inspection report was not available for review during the inspection.

Screen time activity was observed during the children's meal time. Note: Corrected at inspection.

The infant classroom was observed with uncovered electrical outlets accessible to children. Note: Corrected at inspection

One caregiver did not have a current first aid certificate available for review during the inspection.

One caregiver did not have a current CPR certificate available for review during the inspection.

One child with a known food allergy did not have a food allergy emergency plan.

Two of the outlets did not have outlet covers.

Two children with known allergies did not have a food allergy plan on file.

One of the observed crib sheets was not snug fitted.

Two classrooms had two ceiling tiles that needed to be replaced. A light fixture cover also needed to be replaced.

The director did not have transportation training.

During the inspection it was observed that there was a drainage area outside that was uneven, causing a potential tripping hazard for children playing on the playground.

While at the operation it was observed that two 11 month old infants were in their cribs with a blanket. A photo was taken and the blankets were removed while I was present.

While at the operation it was observed that the loose fill on the toddler playground was less than 6 inches deep around the slide and play structure.

I observed children under two watching televison during the unannounced monitoring inspection. Inspector informed the caregiver that children under two do not have screen time. Caregiver turned the television off.

During naptime is was observed that there was not adequate spacing between the mats the children were sleeping on to allow them to walk through the mats without stepping over them. A photo was taken.

Screen time was being utilitzed during mealtime and for children under the age of 2 years old in the 12 months to 18 months classroom. This was corrected when the caregiver turned off the movie.

Based on investigation findings, it was determined that diaper changes are not conducted promptly.

There was no fire drill documented for the month of August.

Based on the documentation obtained during the investigation, it was assessed that there was no emergency medical authorization information.

Two toddler size play structure outside did not have surfacing around or underneath.
