There was not sufficient walkway between a table and cot near the entrance of the room during naptime.

There was uncovered food in the refrigerator.

A cot was blocking a walkway from one area of the room to another. There was no space to walk around.

First aide kit in the transport vehicle was missing the following items: Tweezers and a thermometer. NOTE: Directorwas able to replace the tweezers at inspection.

There were sippy cups and other cups that were not properly labeled.

There was not adequate lighting in the main naptime room during inspection.

Caregiver with no previous experience with children under five years old did not have at least 8 hours of pre-service.

Child had two toys in their crib while they were asleep.

Education obtained outside the United States was not translated into english.

Children records were incomplete in the following areas: Two children were missing the date of admission, one child was missing authorization for water activities, and one child was missing a health statement signed by a medical profesional

Caregiver was missing the following annual trainings: At least six clock hours of the annual training hours must be in one or more of the following topics: (1)Child growth and development (2)Guidance and discipline (3)Age-appropriate curriculum (4)Teacher-child interaction. (c) At least one clock hour of the annual training hours must focus on prevention, recognition, and reporting of child maltreatment, including: (1)Factors indicating a child is at risk for abuse or neglect (2)Warning signs indicating a child may be a victim of abuse or neglect (3)Procedures for reporting child abuse or neglect (4)Community organizations that have training programs available to employees, children, and parents. (e)While there are no clock hour requirements for the topics in this subsection, the annual training hours must also include training on the following topics: (1)Emergency preparedness (2)Preventing and controlling the spread of communicable diseases, including immunizations (3)Administering medication, if applicable, including compliance with ?746.3803 of this chapter (relating to What authorization must I obtain before administering a medication to a child in my care?) (4)Preventing and responding to emergencies due to food or an allergic reaction (5)Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic (6)Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?). (f) The remaining annual training hours must be in one or more of the following topics: (1)Care of children with special needs (2)Child health (for example, nutrition and physical activity) (3)Safety (4)Risk management (5)Identification and care of ill children (6)Cultural diversity for children and families (7)Professional development (for example, effective communication with families and time and stress management) (8)Topics relevant to the particular age group the caregiver is assigned (for example, caregivers assigned to an infant or toddler group should receive training on biting and toilet training) (9)Planning developmentally appropriate learning activities

Personnel files were in complete. One employee did not have a signed pre employment affidavit, notarized affidavit of employment, Signed receipt of operational and personnel policies, or signed receipt of orientation.

There was a dead cat in the corridor leading to the one of the two exits from the outdoor play area.

Two toddler training cups were observed with no labels in dining area.

Food was observed to be left out in microwaves and on toaster ovens past meal times.

Classroom was too dark during naptime. Inspector could not see and had to wait for eyes to adjust.

Disinfecting wipes were observed to be directly adjacent to changing table cushion.

Sleeping cots in use were observed to be without name labels or labeling system.

In the corridor heading to outdoor play area, warped fencing and exposed and protruding nails were observed.

Observed a stick of cheese placed in a basket in the Pre-Kindergarten classroom during the inspection which is required to be refrigerated. Note: The cheese stick was disposed of during the inspection.

The outoor play area contains one of two required exits. One exit leads into the infant room.

The operational policies do not include the following information: - Safe sleep policy for infants from birth through 12 months old that is consistent with the rules in subchapter H of this chapter - Promotion of indoor and outdoor physical activity (A-G) - Procedures for supporting inclusive services to children with special care needs.

The initial fire inspection has not been conducted for the daycare center.

Observed 2 fire extinguishers that were not mounted on the wall during the inspection and was placed on a window seal.

During the inspection multiple issues regarding supervision were observed. - 3 Children were left unsupervised in the dining room area - 1 Child was left in the infant room unsupervised - 2 Caregivers left 9 children in the Toddler room and were not in physical proximity of children

A caregiver stated here were 7 children in care when there were 9 children accounted for during he inspection.

Observed expired milk stored in the refrigerator during the inspection.

Children were observed sitting on tables, and with no structured activities during the inspection.

The first aid kit in the daycare vehicle does not contain scissors or tweezers.

Observed a table in the prekindergarten classroom with a broken leg. The table leg was missing a height adjustment screw and was unstable. The tarp covering the fence in the outdoor play area is torn and a broken hookah hoop was observed. Observed a bottle of brake fluid, a small pepper spray on a key chain and vape pen in the vehicle the operation uses for transporting children.

Documentation showing the dates of when the smoke detector and carbon monoxide detectors were last tested was not available during inspection.

The operation has not designated a qualified director.

Children's files evaluated were missing the following information: - Complete Emergency Contact Information - Complete Authorization for Emergency Medical Information - Signature from parent for receiving the operational policies. - Release Contact Information

One caregivers education documentationw as written in a foreign language that has not been translated to English.

One caregivers file did not contain documentation of a how they meet the minimum education requirement.

Electrical outlets were not covered in the generalized area where children were observed. Note: The director placed an outlet cover over the outlet during the inspection.

The director did not have documentation of current CPR and First Aid Training available for review.
