Two caregivers did not receive annual training in administering medications, food allergies and handling and storing of hazardous materials for the previous training year.

The operational policies does not include the following information: 1. The duration of physical activity (indoor and outdoor), type of physical activity (indoor and outdoor), recommended clothing and footwear, criteria used when weather prevents outdoor play and a plan for physical activity when outdoor play is limited. 2. Instructions on how parents may access Minimum Standards. 3. The correct website for Texas Health and Human Services Commission. 4. Inform parents the center is in a gang free zone. 5. Steps the center will take to increase parent and staff awareness regarding child abuse and neglect, community strategies for coordination between the center and appropriate community organizations. 6. Emergency Preparedness Plan

Of the 10 children's records evaluated, 1 was behind with required immunizations.

Parents have not been provided the contact information for the local Child Care Regulation office.

The operational policies were reviewed and the following information was missing: 1. The hours of operation 2. Procedures for parent to discuss concerns with the director 3. Staff vaccination policy 4. Requirements for outdoor play and physical activities 5. Inform parent's of their rights as a parent of a child enrolled in child care

Of the 10 records evaluated the following was found: 1. Three staff have not completed any pre-service training. 2. Five staff have received a portion of 24 clock hours of pre-service training.

Of the 10 personnel records evaluated, 4 lacked training in one or more of the following topics: emergency preparedness, food allergies, communicable diseases including immunizations, handling, storing and disposing of hazardous materials, administering medications and understanding building and physical premises safety.

Four staff do not have a signed pre-employment affidacait on file.

One director received 17 of the required 30 clock hours of annual training hours for the March 2023-March 2024 training year. The director will need to receive 1 clock hours of make-up training.

Of the 10 personnel records evaluated, one has not completed a First Aid training class.

Of the 10 records evaluated, one has not completed a CPR class that meets the requirements.

The director did not receive training in emergency preparedness and understanding building and physical premises safety.

Of the 10 staff records evaluated, one did not complete 1 clock hour of pre-service training in SIDS, Shaken Baby Syndrome, Understanding Early Childhood Brain Development and Abusive Head Trauma.

This standard was evaluated in regards to a self-report and compliance was not met. The caregiver did not have current CPR training. The caregiver completed a CPR training on-line which is not valid. CPR training must be completed in person.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.
