Based on the information obtained, the director allowed the caregiver to care for children given the circumstances of the caregiver and also allowed the caregiver to continue to care for children prior to receiving clearance that the caregiver is well enough to continue to care for children.

During naptime, there was one caregiver supervising 2 classrooms. However, the caregiver was not properly supervising both rooms at the same time when she came into one classroom leaving the other room unsupervised at a period of time.

An infant caregiver walked out of the infant room, leaving the other caregiver alone with 9 infants with a specified age group of 0-12 months old.

There were bords missing on the playground structure on playground 2. The outer protective rubber mat needs to be replaced and is hanging causing a tripping hazard. The pipe at the infant playground needs to be completely covered. A board near the back fence needs to be added for more protection for the children.

There were multiple bottles that were labelled with the first and last name of the infant.

There were 12 infants, specified aged of 0-11 months old, with 2 caregivers in the infant room. The caaregiver walked out of the infant room on several occaisions leaving one caregiver with 12 infants.

There were 2 infants,aged 4 and 6 months, with 6 other children who were between the ages of 18 months and five years old. This was corrected when the group was separated.

The employee list was not validated by January 15th as stated in minimum standards.

There were two individuals present at the operation prior to having a completed initial background check with fingerprints. Both individuals left the operation.

The information to promotion of indoor and outdoor physical activity was not inlcuded it the operational policies.
