CCR did not observe a diaper changing table in the toddler classroom where the infant was being cared for. Instead, the infant caregiver was using a sleeping mat labeled as a diaper changing mat, which had a hole and is no longer non-absorbent, to change the children's diapers.

Within three minutes, CCR observed an infant strapped in a baby swing, sleeping in the restrictive device.

CCR observed the director caring for 13 children, with the youngest being two months old and the oldest eight years old.

The operation has an open yard where the children's playground is not enclosed. CCR observed several hazards accessible to children, including fire ant killer, wet wipe boxes collecting water, unused sleeping cots, ladders, and gardening soil.

CCR did not observe the recent licensing inspection posted.

A child who was diagnosed with a food allergy by their healthcare provider does not have an EpiPen stored at the daycare.

The director left the children unsupervised in the classroom to open the door for CCR to enter the building.

Children were served a beverage with 8 grams of added sugar that consist of 15% juice for PM snack. Note: this was corrected at inspection when the director removed all sugar added beverages.

The director is missing 6 hours annual training in management/leadership/supervision.

The operation changed the operating hours, is opening at 5am and did not make notification of the change.

4 electrical fans were sitting on the floor within reach of children.

The sanitation inspection was conducted on 3/4/25 and expires on 7/20/25. The report indicates non-compliance.

The carbon monoxide detector indicated 1 "chirp" per minute, indicating to replace the battery.

Two groups of children were not supervised during naptime when the caregivers stepped out of the classroom.

1 sleeping mat was being used by a child that was torn with exposed foam, making it impossible to sanitize. Note: the caregiver threw it away and replaced with a new one.

The director failed to operate in compliance with minimum standards and failed to ensure all employees comply with the minimum standards.

The operation served an identified food to a child with food allergies, resulting in the use of an epi-pen and ambulance being dispatched.

There is the presence of rodent feces in the kitchen cabinets with a large hole in the wall inside the cabinet that is the suspected entry/exit of rodents.

The operation failed to complete an incident report and share with CCR upon request.

The operation failed to make notification to CCR as soon as possible, but no later than two days after an Injury to a child in your care that requires medical treatment by a health-care professional or hospitalization.

4 out of 4 infant bottles and cups are missing the required name label.

Outdoor play equipment on the children's playground has two screws protrudiung from the structure.

4 out of 4 infants is missing the daily report. Daily reports are made on Brightwheel, however the caregiver was not able to access the daily report for review by CCR.

The director needs to complete 2 hour transportation training and keep the training certificate in the staff file and made available to CCR for review.

The hand washing sink in the diaper-changing area in the infant room is out of service.

2 out of 2 infants not yet on table food requiring written feeding instructions were outdated.

The children have not been outside since June 2024 for outdoor physical activity.

Part of the fence is laying on the ground and needs to be stood back up or replaced.

2 active ant beds are present on the playground near the active play equipment.

The toddler room floor has multiple areas of ripped/ torn vinyl flooring that needs repair or replacement.

A fire inspection has not been completed since October 27,2022..

A large spiny cactus has grown up from the neighbors yard along the playground fence, made accessible to children during play.

One feeding instructions sheet for an infant on bottle food was last signed by the parent in August 2022.

Two tricycles, one plastic see-saw, and one child chair located in the playground, were damaged with sharp edges. Note: this was corrected at time of inspection when the director removed it.

Multiple active ant beds in the playground were observed.

The use zone for the exit of the slide is measured at 3 1/2 ft from the barrier.

The loose fill is measuring at 3 inches out of the 6 inches.
