The operational policies were incomplete and/or lacked all the required information, reference standard 747.501. The operational policies lacked 747.501(6, 7, and 10).

The provider did not have any training certificates available for review for the following training period: 03/31/24-03/31/25. The training hours completed for 03/31/25-03/31/26 are 26 of the 30 clock hours for this training period. The required annual training on the following topics had not been completed by caregiver. The required training topics are 1. emergency preparedness 2. communicable diseases to include immunizations 3. administering medication 4.preventing and responding to emergencies due to food or an allergic reaction 5. building and physical premises safety and 6. handling, storing, and disposing of hazardous materials.

One child's file lacked an immunization record.

The documentation available for review did not indicate vaccinations had been administered to the pet in the home.

Monthly fire drills have not been conducted. The last fire drill documented was 10/27/25. Two fire drills must be completed by 01/15/2026.

The most current inspection report was not posted. The inspection report posted was from 2022 not 2025.

The Children's Products form had not been reviewed annually. The last date it was reviewed was 03/22/2022.

One child's filed lacked the release information.

It was noticed that one child in care did not have a preschool health statement.

It was observed that the date of admission was missing from three children's records. Note: The caregiver added the admission date to the records during the inspection.

The first aid kit did not contain adhesive tape , tweezers and scissors.

It was noticed that the school's name and phone number were missing from the records of two school-aged children.

One child in care did not have up to date immunizations required by TX State Immunization law.

The inspector observed the home has fire extinguisher rated 4-A, 60-B: C

There was no current cpr and first aid training available for viewing.

Based on the investigation information obtained there were sufficient evidence to conclude there were no plan to handle transportation emergencies available for viewing. Note: The caregiver stated she will no longer transport children.

Based on the investigation information obtained there were sufficent evidence to conclude the operation provided transportation, but did not notify licensing that transportation was being provided. Note: The caregiver stated she will not be providing transportation services any longer.
