A child of age 2yrs was observed getting diaper change in a classroom by a caregiver were other children were present.

Caregivers folders was observed missing several required documentation.

Feeding instructions for 9month could not be oberved during inspection.

A infant child in care enrollemnt folder and several children files were observed to be missing required docuementation.

Caregiver did not know age of child in classroom during inspection.

There were two children placed in a high chair as result of them actibt "bad".

The sheets in the cribs in the infant were loose and not snug fit.

One of the five caregivers files reviewed did not have documentation showing the required completed 24 hours of annual training.

A caregiver was present and caring for children with an ineligible background check status. The operation was notified of the ineligible status on 5/13/2025.

The center did not report the number of employees that left employment in 2024 by the required date of 1/15/25.

During the walk through in the Infant room 5 children ages 2-14months observed in the classroom without a caregiver supervising children in care. The caregiver came from the Toddler room back into the infant room.

Children were observed napping on mats closely to each other with no walk space on either sides of their mats. Caregiver were unable to walk between cots. CI: Caregiver rearranged the children sleeping arrangement.

2 children observed ages 2yrs old sleeping in high chairs during nap time. CI: caregiver removed children and placed them on sleeping mates.

Children were observed sleeping blocking the entrance and exit doors of the classroom. CI: Caregiver rearranged the children blocking the exit doors.

Caregivers annual 24hr trainings could not be observed.

Caregivers cpr licenses was observed to be expired. 3 caregivers cpr licenses could not be observed.

Caregivers folders were observed to be missing several documents and trainings.

First aid licenses was observed exipred for 1 caregiver 3 other caregivers cpr/first aid licenses could not be observed.

4 baby walkers with wheels were observed with infants in them.

4 of the 5 children records observed did not have a completely filled out admission form, immunizations and health statement information.

All four caregivers folders were observed to be missing several documents.

Based on information obtained throughout the investigation, it was found that one operation staff may supervise two classrooms during pick-up time. It was found that one caregiver stands in the doorway to supervise 2 age groups.

Based on the attendance log from 09/28/2021-09/30/2021, a total of 6 infants were signed in, 0-12 months of age with one caregiver.

3 caregivers did not have a CPR/First aid license to observe.

2 caregivers with hire dates of 06/02/2021 and 07/13/2021 did not have preservice training and orientation.

Director fail to ensure a caregivers left in charge of groups of children without the required pre-service training, SIDS, Infant, and CPR/ First Aid. I observed 1 of the 4 caregivers to have CPR/ first aid licensing and preservice training. 1 of the 4 caregivers folders could not be observed.
