There are several indicator lights on the dashboard including the change engine oil and service tire. The back break and turning signal lights are not working properly.

Two children with known food allergies did not have a food allergy emergency plan available for licensing to review.

During the inspection, caregivers were unable to identify who was in charge of the operation. When I asked one caregiver who was responsible, they identified another caregiver. However, that caregiver stated that they were not in charge.

Observed expired antiseptic solution in the first aid kit. NOTE: This was removed and replaced during the inspection.

I observed chipped paint on the walls in several classrooms.

During the walk through a caregiver was heard yelling to a child in the PreK classroom, " If you don't stop screaming, you will not get anything!" This was during lunchtime.

A caregiver in the PreK classroom was heard yelling from the back of the center as we entered the front of the building.

Information was obtained during an investigation inspection that a caregiver responsible for the school age children uses their personal cellphone device to take phone calls while children are in care.

During this investigation, it was determined a caregiver responsible for the school age group is using profane language while children are in care.

During an investigation, it was determined that children were engaged in screen time activities during rest times that were not related to the planned activities to meet educational goals, not age appropriate, and contained advertising or violence.

During an investigation, it was determined that caregivers were leaving children unattended.

The activity plans were not posted in the classrooms. Note: This was corrected at inspection.

Children in the toddler classroom were observed jumping on chairs, sitting on a table, running around the classroom, and pulling other children across the floor on chairs.
