There was child takin a nap and laying in front of the exit door area. This was correct when the child was moved from in front of the exit door.

There were 6 infants with one caregiver in the infant room. This was corrected when another caregiver came into the classroom with the infants.

The director and one caregiver are not keeping a record of the days and hours worked. All staff must document their work hours.

Of the 10 children's records evaluated, six lacked the date of admission to the child care center.

Of the nine personnel records evaluated, four did not receive the required number of annual training hours. Caregiver A: For the August 2020-August 2021 training year, the caregiver received 6 clock hours of annual trainng. The caregiver will need to receive 18 clock hours of make-up training. Caregiver b: For the February 2020-February 2021 training year, the caregiver received did not receive any clock hours of annual trainng. The caregiver will need to receive 24 clock hours of make-up training. Caregiver C: For the Macrh 2020-March 2021 training year, the caregiver received 12 clock hours of annual trainng. The caregiver will need to receive 12 clock hours of make-up training.

Of the nine personnel records evaluated, three lacked a hire date.

The director did not receive the required 30 clock hours of annual training for the August 2020-August 2021 training year. The director will need to receive 30 clock hours of make-up training, The make-up training hours will not count towards the current training year.

A list of children being transported is not in the vans.

The amount of food is not being written on the infant daily report sheets.

The center does not have a current health inspection. The date of the last inspection was 9/21/20.

The infant written feeding instructions for three infants has not been updated within the last 30 days. The infants written feeding instructions must be reviewed, updated and signed by the parent every 30 days.

The fire extinguishers in the van are both on red which means they are empty. New fire extinguishers are needed for the van.

The required emergency medical transport and treatment authorization forms are not on the vans used for transportation.

Daily report sheets are not being completed for infants that are 12 months and older. A daily report must be provided to the parent for all infants, An infant is defined as a child age 0-17 months.
