I observed two infants less than 12 months covered with blankets in their crib. This was corrected when the director reminded the caregiver the children can not have anything in the crib besides a fitted sheet. The caregiver removed the blankets.

Three children emergency medical forms were not in the transportation binder when licensing staff reviewed the binder.

Two children's files were not avalible for review when requested.

The operation did not provide the sign in and out sheet for August 7th.

The notification to parents did not include the time the children would return to the operation from the field trip.

The operation did not have a signed permission slips for the field trip.

According to DFPS investigation findings there were statements made that caregivers are utilizing corporal punishment and statements were made that a caregiver uses threats of corporal punishment.

There was no transportation emergency plan in the transportation folder.

One child who is transported did not have emergency medical consent in the transportation vehicle.

One child who is transported did not have transportation consent to and from school.

The required emergency contact information for each child being transported is not in the vehicle used for transportation.

The primary caregiver has not completed a transportation safety training.

The primary caregiver received 6 of the 30 clock hours of required training. The primary caregiver will need to receive 26 clock hours of make-up training. The make-up training hours will not count towards the current training year.

One caregiver did not receive training in the following topics for the October 2022-October 2023 training year: emergency preparedness, administering medications, communicable diseases including immunizations, understanding building and physical premises safety and handling, storing and disposing of hazardous materials. The caregiver will need to receive make-up training hours that will not count towards the current training year.

The home is not equipped with the proper size fire extinguisher.

The First Aid Kit lacked a Guide to First Aid and Emergency Care.

The required authorizations for emergency medical care for each child being transported is not in the vehicle used for transportation.

Two personnel records lacked a notarized affidavit.

Of the four children's records evaluated; the following information was missing: 1. Two lacked a shot record 2. Two lacked the address of the emergency contact 3. One lacked the name, address and phone number of an emergency contact other than the parent 4. Two lacked a health statement 5. One lacked the name and phone number of the school the child attends 6. Two lacked the required information regarding special needs 7. Two lacked the required information regarding water activities

This standard was evaluated and compliance was not met. The following information is missing from the operational policies: 1. The duration of outdoor play 2. The duration of physical activities 3. Safe sleep policy for children 12 months old and younger 4. Informing parents there is an open door policy for them to observe their child's activities

There were uncovered outlets in the room used by the children.

A child's hands were not washed after a diaper change. This was corrected at inspection.

Children under the age of 4 were served whole grapes. Whole grapes are a choking hazard for children under 4 years old. This was corrected at inspection.

The child care home is not equipped with a carbon monoxide detector.

There is no documentation of drills available for review.
