One caregiver was observed to be present and in care of children without a background check on file. NOTE: Corrected at inspection, the background check was submitted during the inspection

Records for 10 staff was reviewed, 7 caregivers hired after 2023 did not have a preemployment affidavit on file.

Based upon the information obtained, multiple incident reports were not signed by the director or person in charge during the incidents that occurred.

2 infants not yet on table food did not have updated feeding instructions. One was last completed 10/28/22 and another was last completed 12/27/22

During the inspection, it was learned that the operation failed to recent case a COVID-19 within 48hrs.

Based on the information obtained during the investigation, the operation failed to demonstrate good judgment when they deleted an incident report from a caregiver who witnessed a child have a seizure in care and then created a new report to state the child hit their head.

During the inspection I observed the Early Preschool II classroom on the outdoor playground. Broken zip ties, plastic, and other items accessible to the children was observed on the playground. Note: The director and inspector removed the items from the playground.

The caregiver in the Preschool Pathways classroom stated eight children were present. The number of children were counted and nine were present in the room.

Three of ten personnel files reviewed needed skills based pediatric CPR training.

Two of ten personnel records reviewed needed documentation showing how the caregivers meet education qualifications.

The operational policies reviewed did not include the following policies: -Suspension and expulsion of children -Instructions on how a parent may contact the local Licensing office, access the Texas Abuse and Neglect Hotline, and access the HHSC website -Provisions to provide a comfortable place with an adult sized seat in your center or within a classroom that enables a mother to breastfeed her child. -Gang free zone -Preventing and responding to abuse and neglect of children, including: -Required annual training for employees Methods for increasing employee and parent awareness of issues regarding child abuse and neglect, including warning signs that a child may be a victim of abuse or neglect and factors indicating a child is at risk for abuse or neglect -Methods for increasing employee and parent awareness of prevention techniques for child abuse and neglect -Strategies for coordination between the center and appropriate community organizations -Actions that the parent of a child who is a victim of abuse or neglect should take to obtain assistance and intervention, including procedures for reporting child abuse or neglect

Three of ten children's records reviewed needed updated immunization records.

Medication stored for one child was not in its original container.

During the investigation it was determined that supervision was not maintained even though the caregiver was in the room they were not able to intervene to prevent a child from being injured.
