Records were evaluated for ten children enrolled and the following was determined: Emergency contact information is needed for two children enrolled. A health statement is needed for eight children enrolled. The admission date is needed for four children enrolled. Current immunization records are needed for eight children enrolled.

Two caregivers completed online pediatric CPR training.

Infant feeding instructions are needed for two children in care.

The last fire inspection was completed 01/02/2025.

The operation did not notify Child Care Regulation of transportation services.

The first aid kit in the van used to transport children did not have a working thermometer. Note: This was corrected during inspection when the director added a working thermometer to the first aid kit.

During today's inspection a three year old child was transported without a child passenger safety seat. Note: The director added a booster seat in the vehicle during inspection.

The antiseptic wipes in the van used for transporting children expired 11/2020. Note: This was corrected during inspection when the director added antiseptic wipes that were not expired.

The transportation safety certificates did not specify the length of the training.

The transportation binder was reviewed and it did not include emergency medical transport and treatment authorization forms for the seven children the operation transports.

Upon arrival I encountered two caregivers and one kitchen personnel. All were asked who was in charge in the directors absence and they stated the assistant director who was not present. The staff were asked who was in charge at that moment and there was no qualified caregiver designated to be in charge.

An Emergency Preparedness Plan consistent with the requirements listed in minimum standards 746.5202 has not been added to the operational policies.

The Emergency Preparedness Plan reviewed did not have procedures for relocating children who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing impairments.

The caregivers in the Discovery Preschool room was unaware of the number of children responsible for. A caregiver stated 15 children were in the room, however 16 were observed in the classroom.

The Emergency Preparedness Plan was reviewed during inspection and the following is needed: Instructions for evacuating and relocating children who are under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency. The staff responsibility in a sheltering/lock-down emergency for the orderly movement of children to a designated location within the center where children should gather. The alternate shelter away information provided to parents needed updating.

Three staff members need training in Understanding early childhood brain development.

The Emergency Preparedness Plan did not include information about how the staff will continue to care for the children until each child has been released.

During today's inspection I observed a group of children napping in the Discovery Preschool class ranging from ages 14 months old-3 years old. The oldest child was more than 18 months older than the youngest child.

The Emergency Preparedness Plan did not include information about how the operation will evacuate and relocate with information such as authorization for emergency care for each child in care.

During today's inspection I observed children laying on numbered nap cots in the Pre-K room. The number/child assignment chart posted was not followed. A number/child assignment list was not available in the Preschool room. Note: This was corrected during inspection.

The Emergency Preparedness Plan did not include How the operation will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.

Updated immunization records is needed for three children enrolled.

During the follow up, there were still two screws missing on the climbing equipment in the DPS classroom causing the equipment to be unstable. There are still uneven cracks on the outdoor play area that also pose a tripping hazard for children who are playing on or near the surface.

A dead bug was observed on the floor by a shelf in the classroom that has not been disposed of. Note: The opreation disposed of the dead bug during the inspection.

Bottles in the Infant Room were missing a label. Note: The provider labeled the bottles during the inspection.

1 of the caregivers in the infant room stated there were 7 children in the classroom when there were 8 children accounted for.

Mangos were served with lunch instead of pears as listed on the daily menu. Note: Provider corrected this at inspection by documenting the substitution on the provider's app.

1 staff file evaluated did not contain the full 24 clock hours of annual training. An additional 8 hours is needed.

2 staff files evaluated did not contain complete documentation of orientation, Signatures from the trainer were missing.

An incident report was not shared with a parent for a child who was injured while in care.

During the inspection a 2 month old was observed sleeping in their crib while the drop gate was down. Note: The caregiver secured the drop gate during the inspection.

The annual fire inspection has not been conducted since November 15th, 2022.

An 11 month old infant was observed in their crib with a baby doll. Note: The provider removed the doll from the crib during the inspection.

Based upon the information obtained, the special care needs statement was incomplete.

A caregiver was observed serving food to children on the bare table with no sanitary napkin.

During the follow up inspection, the operation has received repeated deficiencies and is not maintaining compliance with minimum standard requirements.

A caregiver stated there were 13 children in the classroom. After counting the children, there were 14 children present.

A caregiver did not have children to wash their hands after diapering. Note: This was corrected at inspection when the caregiver had the children to wash their hands.

Bottles were not labeled with the childs first name and last initial during the inspection.

During the inspection a classroom smelled of urine.

There was no toilet paper in the children's restroom during the inspection.

Based upon the documentation obtained, the incident reports were not signed by the provider.

During the inspection, children were observed fighting and caregivers did not intervene to ensure safety.

Based upon the information obtained, there is sufficient evidence to support a violation of this standard.

The annual sanitation inspection has not been conducted since July 26th, 2022.

The operation has not conducted the 4 required sheltering and lock-down drills within the last calendar year.

During the inspection a loose bolt to the bottom of a book shelf was loosened and the toilet roll dispenser in the restroom was broken. The bolts on the boarder of the playground are not secured down and pose a tripping hazard.

During the inspection brown stains were observed on the ceiling tiles in the classroom.

A child was observed in a classroom alone with no caregiver standing at the sink. Note: Sueprvision is criticial to the safety and well being of children in care. Caregivers must have visual and audio awareness to maintain an adequate level of supervision.

Infant feeding sheets were last updated June 14th, 2023 and one sheet did not contain a date.

The medicatication log did not contain the name of the employee who administered the medication or the date it was administered.

Children did not have their hands washed after coming inside from their outdoor activities during the follow up inspection.

During the follow up inspection 2 caregivers were observed with direct access to children. The operation has not received an eligibility determination from CBCU stating the caregivers can be present. Note: Discussed with the director that caregivers must have an active and eligible background check before being allowed direct access to children in care. Caregivers left the operation during the inspection.

A child in the two year old classroom was grabbed by the upper arm and sat down in their chair. Note: It was discussed with the provider the proper way to direct children to sit down by holding and picking them up from underneath their arms.

Documentation of current liability insurance was not available for me to review during the inspection.

During the inspection the Pre-Kindergarten classroom did not have 3 children signed in. Note: This was corrected during the inspection when the caregiver signed the 3 children in.

During the inspection a child was observed sleeping in a crib with a pacifier that contained an attachment piece to their clothing. Note: This was addressed during the inspection by explaining to the operation that pacifiers with attachments are a strangulation and choking hazard.

During the inspection, a black cap was missing from a play structure on the younger children's playground.

Based upon the information obtained there is sufficient evidence to support the maintenance of the building is not being maintained.

Based on the information obtained, caregivers did not wash their hands properly after changing diapers.

A caregiver does not have current CPR and First Aid training. The caregivers training expired on April 26th, 2023.

During the DFPS Investigation Inspection, soiled diapers were left in an uncovered trash can.

Based on information obtained, there was sufficient evidence to support the operation did not notify Child Care Regulation of a communicable disease.

A caregiver who did not have a cleared background check was present at the operation and had access to children in care.

The operation does not have a current annual fire inspection as it expired on 9/17/2022.

There was no documentation showing a severe weather drill was conducted this year.

One out of five employee personnel files evaluated was missing an affidavit.

One out of five employee personnel files evaluated did not have education credentials translated into English.

Four out of ten children's files evaluated are missing current immunization records.

In the preschool discovery and toddler classrooms, the naptime cots were not labeled. Note: Corrected when the cots were labeled with the children's names.

One out of five employee files evaluated was missing five out of twenty hours of annual training for the last annual training year.

Based on information obtained, there was sufficient evidence to support rodents were observed at the operation.

Two caregivers with pending background checks were present at the operation and had access to children in care.

Based on information obtained, there was sufficient evidence to support children were not adequately supervised as three toddlers walked out of the exit door of their classroom and were on the playground unsupervised.

Based on information obtained, there was sufficient evidence to support a caregiver physically disciplined a child.

Several nap cots were observed to be placed behind shelves. From the caregivers position in the room, the children could not be seen.

In the pre-k bathroom, puddles of water was observed on the floor.

A child was observed laying on a stack of cots, the child then stood on the cots several times and attempted to climb on a cubby above. During naptime several children were observed running around the classroom.

Two toddler classes combined and the caregivers in charge were not aware of the ages of the children they combined with. They caregivers were asked several times of the ages of the children they combined with and could not provide an anwser. When a third caregiver walked into the room, they informed Licensing the children belonged to that caregiver and knew the children's ages.

CPR training was reviewed for 5 staff including the director. Training for the director and two additional caregivers was received online with no hands-on training.

Records were reviewed for 5 staff including the director. First-aid training for one caregiver was non-pediatric and two employees did not have current first aid training.

Two caregivers with a hire date of 6/1/21 and 8/16/21 did not have documentation of completed orieintation training.

The director did not have current training in SIDS, shaken baby and early brain development.

A child was observed sleeping in front of an exit door.

Several training certificates observed in the director's file and an infant caregiver's file did not have the trainers qualifications listed.

Training certificates reviewed for four staff did not show that the staff had current training in Early Brain Development.

An infant who is not yet on table food did not have updated feeding instructions. NOTE: The instructions were last updated 7/3/21

Several times throughout the inspection, parents were observed being stopped at the door, it was learned that parents are not being allowing inside of the building due to the operations policy.

Based on the information obtained during the investigation, Covid cases are not reported.

Based on the information obtained during the investigation not all parents were notified of positive covid cases. For one of the positive cases, parents were not notified within the required time frame.

Based on the information obtained during the investigation, there is sufficient information available to support parents were not notified of a situation that placed children at risk.

Based on the information obtained during the investigation, there is sufficient evidence to support allegation of ceiling leaking

Based on the information obtained during the investigation there is sufficient information to support the allegation regarding pest. Dead pest and rodent droppings was observed in several areas of the operation, Pest control services made several recommendations for building maintenance that was not followed.

Based on the information obtained during the investigation, there was a shooting in the area that placed children at risk and Child Care Regulation was not notified.
