An individual with a pending background check was observed with another caregiver caring for school-age children in the cafeteria area. This was corrected during the inspection when both individuals vacated the premises and another caregiver was put in charge.

The playground does not have six inches of surfacing

A children's bicycle was observed cracked and had sharp edges that can poke or cut a child. This was corrected during the inspection when the director discard the play equipment

The fire inspection report was last issued on 5/12/2025 and a current fire inspection has not been completed.

Two live ant beds were observed on the playground

The directors CPR first aid has expired and has not been renewed for pediatric first aid with rescue breathing and choking.

The director does not have a current CPR first aid and it needs to be renewed

The last four fire drills exit time was not within 3 mintues.

There were seven children with one caregiver in the infant classroom. The youngest child was ten months and the oldest child was twenty-four months. The specified age group was seventeen months.

Two children were observed strapped in a chair for time out. The first child was buckled in the chair because the caregiver stated they run around and will not stay still while standing in time out and the second child was strapped in the chair because they threw a doll house.

A broken bird feeder was observed on the playground structure. The bird feeder contained broken pieces and sharp edges and was assessable to the children. This was corrected during the inspection when I observed the caregiver remove the broken bird feeder.

A nail bolt was observed sticking out of the black boarder on the playground structure.

While playing outside on the playground, children had access to a water table that was filled with water. Some of the children were observed splashing and playing in the water and it was confirmed by the caregiver that it had rained and the children were not supposed to be playing in the water. This was corrected during the inspection when I asked the caregiver to pour out the water. The caregiver did remove the water and directed the children to another activity.

A wooden boarder was observed broken and two nails were exposed and assessable to children.

The operation had combined infants with older children during closing when there were 13 children present. Only children 18 months and older can be combined for a reduced ratio when there are more than 12 children present.

The operation did not have a current fire marshal inspection.

One child's parent stated on the admission form that they had a food allergy, but did not provide a food allergy plan.

The loose fill in the playground use zones was less than two inches.

A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

The most recent sanitation report available to licensing was expired.

Caregiver in charge did not know how long the director would be absent.

Caregiver in charge did not have access to the records required to initiate an investigation.

There was sitting water in a mop bucket, left in hall way near the infant and toddler classroom with approximately 4 inches of water. This was corrected by caregiver in charge by moving the mop bucket into the kitchen.

There was a large hole in school age classroom floor. There was also a small hole in playroom wall, and the trimming was detached from the wall in multiple classrooms.

The director did not have attendance records available for review.

An infant was left in car seat for more than 15 minutes. This was corrected when the infant caregiver removed the infant from the car seat.

Recent pest control records were not available for review.

There was sufficient evidence to suggest that the heater is not functional.

There was sufficient evidence to suggest that the operation was not kept sanitary and free from roaches and mice.

A 17 month old was observed in care in a classroom with three and four year old children.

A four year old child was observed napping in a high chair in a room alone.

A video was observed playing on an ipad while a four year old napped within sight range.

This deficiency was re-cited when, upon inspection, the operation could not provide evidence that the fire marshal had conducted an inspection.

Upon reinspection, four children under two were observed watching a video on an ipad. This was corrected when the caregiver took the children to another room.

One of eight children's records did not have up to date immunizations.

A caregiver was observed leaving two infants in a room to get a mop from down the hallway.

Three children under two years old were observed watching a video on an ipad.

A two year old was left sleeping alone in a crib while the caregiver tended to another child in an adjacent room for approximately five minutes or more. This was corrected when I instructed the caregiver in charge to have the caregiver move all supplies into one room.

The operation did not have documentation of fire drills for January or February 2024.

The operation did not have an up to date fire inspection.

The fire extinguishers had not been checked in the months of January and February.

A review of the Employees Who Left Employment in 2023 data, revealed that the center had not reported the number of employees who left employment in 2023 by January 15, 2024, as required.

Two infants ages 10 and 14 months were observed sleeping with bottles in their mouths in restrictive devices for approximately 20 minutes. This was corrected when the caregiver was told to take the bottles away and put the infants in cribs.

The caregiver in the Pre-K room did not know the age of the oldest child or the youngest child and did not have the information accessible anywhere in the room.

Records could not be produced for the director's attendance.

The director has not been present for the last three licensing visits. The staff could not say when she was expected back and there were no records available to indicate when she was last present at the operation.

Two infants aged 10 and 14 months were observed sleeping in restrictive devices for approximately 20 minutes. This was corrected when the caregiver was informed that she must place them in cribs.

Children under the age of two were observed watching TV. This was corrected at inspection when the caregiver turned off the television.

Two children with special needs were placed in high chairs for an extended period and not given activities that allowed them to participate in the planned daily activity in the least restrictive environment possible.

A caregiver was working in the preschool room without an active and eligible background check. This could not be corrected at inspection because there were not any staff to provide coverage for the classroom.

Upon my arrival, the director was not present and no one was aware of being left in charge.

Children under the age of two were observed watching a video on a tablet. This was corrected when the caregiver turned the ipad backwards and used the tablet for music instead.

School age children were allowed to come and go from the building to use the restroom and get water out of sight and hearing of caregivers.

The van did not contain emergency medical transport and treatment authorization forms for each child being transported. This was corrected when the missing information was added to the binder.

The first aid kit did not contain tweezers.

The twos and threes classroom was watching cartoons while eating breakfast. This was corrected on inspectioon when the t.v. was turned off.

The van did not have a copy of parent s names and telephone numbers and emergency telephone numbers for each child being transported. This was corrected when the missing information was added to the binder.

The records of two children did not contain admission dates. The record of one child did not contain the address for the emergency contact person. The record of one child did not contain field trip permissions. The reccords of two school aged children did not contain information about the name and phone number for the school attended.

The vaccine records of two children were not updated.

One child did not have a health statement signed by a parent or physician on file.

Daily sheets with diaper change times are not sent home with the infants daily.

The operation has ordered an extinguisher and it waiting for it to arrive. She expects to receive it by Monday 4.25.22

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

There was a bottle of Lysol on the sink in one of the children's restrooms. NOTE: This was corrected at inspection when director removed the bottle and stored it out of reach of children's reach.
