One child did not have release of children information in their file. This was corrected when mom arrived at the facility and added release information.

Bottle of Clorox aerosol was sitting next to children within reach.

Caregiver was observed using Clorox aerosol spray to disinfect bedding in the direction of and in the presence of children.

The diaper changing table was not sanitized following a diaper change.

Nap mats used are not labeled with the child's name or number. According to staff, mats are placed down and sleeping spots are identifed once the child's blanket is placed on the mat.

6 children's records were reviewed and observed to be mssing the following documentation: 3 were missing completed emergency contact 2 were missing contact information for release persons 1 was missing the child's home address 1 did not have emergecy medical authorization.

The bottom support of a crib was observed to be broken

Several exits doors designated as emergecy exitss and evacutation routes were missing a posted diagram.

The 2 year old classroom was observed to be overratio. There were 26 children between 21m-2yrs old. The Sepcified Age Group is 2yrs old. The classroom was over ratio by 4 children

Training records for 9 staff including the director was reviewed. All staff received more than 46-64 hours of training within a 24 hour period. Training certificates were copied and whited out to add a new caregiver's name. Original certificates were not available. Some of the certificates obtained for the director listed the director as the trainer and participant.

2 infants were observed to be sleeping in cribs with bibs around their neck.

4 play yards were observed to have loose fitted sheets

Documentation of manufacture instructions for a climbing structure used for the 2 year old group was not available during the inspection. According to the manufacture, the equiment is designed for children 6-10 yrs old

A caregiver was observed using disposable gloves during a diaper change. The caregiver did not remove the gloves after the diaper change. The caregiver was observed leading children to centers and picking up books before being instructed by CCR to remove the gloves and wash their hands.

2 play yards in the infant room were not observed to have mattresses fitted for the play yards.

Two doors designated as emergency exits were observed by be blocked by baby gates.

Three out of five children record evaulated needed a health statement.
