Three staff folders reviewed during the inspection were missing the required information( training hours, photo identification, notarized affidavit, hire dates, background checks).

There was no record of the required 30 hours of training for the primary caregiver available to review during the inspection.

A total of 13 out of 30 required hours of training for primary caregiver was obtained via email on 6/13/25. The primary caregiver still lacks 17 hours in training hours. This assessment was approved by CCR supervisor Amy Johnson. The compliance date of 6/20/25 was changed to 6/27/25.

The operation was observed out of ratio, when one caregiver was observed caring for nine children ages 20 months- 3 years old, and two 4-year olds during the inspection. This was corrected during the inspection when a second caregiver was observed returning to care for children.

This was recited when a 17 year old child of a caregiver was observed present in the room with the children without a valid background.

A caregiver was observed caring for children during the inspection without an approved background check. The caregiver was observed leaving the operation.

The operation was observed out of ratio, when one caregiver was observed caring for nine children ages 18 months- 3 years old, and two 4-year olds during the inspection.

A electric fan was observed not mounted out of the reach of children during the inspection. This was corrected during the inspection when caregiver was observed unplugging, and removing the fan from the shelf out of the reach of children.

Epi-pens for two children in care were observed not labeled with child's full name and date it was brought to the operaqtion during inspection.

Ten children records reviewed were missing information on any adaptive equipment provided for child including instructions for how to use the equipment during the inspection.

Ten children files reviewed were missing information on symptoms or indications of potential complications related to a physical, cognitive, or mental condition that may warrant prevention or intervention while child is in care during inspection.

Ten children records reviewed were missing information on reasonable accomodations or modifications during the inspection.

Ten children records observed missing permssions for participation in water activities including whether child is able to swim without assistance. during inspection.

Electric fans were observed not mounted out of the reach of children during the inspection.

Operation was observed extremely warm, and not well ventilated with thermostat showing temperature of 81 degrees during inspection

There were no individual towels or disposable towels observed in restroom for handwashing and drying during inspection.

Fitted sheet in crib was observed too loose and not snuggly/tight fitted.

The use zones for stationary equipment was observed less than 6 feet in all directions.
