During a walk through of the building and grounds, it was observed on the outside playground exposed nails coming up from the wooden boarder. This was brought to the person in charge attention and quickly taken care of.

There were several spots on the playground border that were broken and showing screws and hazardous to children.

The operation did not ensure supervision when a child's whereabouts were unknown for a period of time. The operation did not have physical proximity and auditory or visual awareness of each child?s on-going activity.

During the inspection trash cans in multiple rooms that had used diapers inside and were in reach of children had no lid.

During the inspection I observed multiple outlets in multiple rooms with children that did not have any electrical outlet covers on. This was corrected at the inspection when the operation placed the covers on the outlets.

During the investigation it was found that a caregiver slapped a child on the forehead as a form of punishment.

On 3/29/23, one staff member was supervising 22 children 18 months and older for one hour and thirty-four minutes. Eleven children were between the ages of 18 months ? 3 years, six were between the ages of 3 years ? 5 years and five children were school age children.

This standard was reviewed in relation to the reported allegation and found to be deficient. The operation did not notify CCR that a child sustained an injury while in care that required medical treatment.

This standard was reviewed and there was a violation of minimum standards. The schoolers caregiver did not use good judgement when allowing food to sit for 5 minutes on the cart while the class cleaned up the tables.

During inspection, a staff member who cooks and cleans the operation did not have a background check. The director ran a background check while I was in the operation.

A child was injured in care on 6/22/22 and required treatment from a medical professional for a fractured arm. The operation did not notify licensing as required.

This standard was reviewed in relation to the reported allegations and found not to be in compliance. A baseboard in a classroom was observed to be a hazard.
