Children's House at Jourdanton

Eight children were served their afternoon snack without a sanitary holder. The children were observed eating crackers off of a bare table. This was corrected at inspection when it was addressed with the caregiver.

One out of ten employee records evaluated were lacking 9.5 annual training hours.

One out of ten child records evaluated was lacking an emergency medical authorization.

Five out of ten children's records evaluated were lacking up to date immunization records.

A caregiver was observed crossing over children who were laying on their nap mats due to an inaccessible walkway between napping equipment. This was corrected at inspection when it was addressed with the caregiver.

A caregiver did not wash an infants hands after a diaper change. This was corrected at inspection when it was addressed with the caregiver and the infants hands were washed.

During the inspection: There was one caregiver supervising two separate classrooms during naptime. The caregiver was observed standing in one room, leaving the second room unsupervised. Regarding this investigation, a child was able to unlock/unlatch a gate and stepped outside of the fenced area while outside.

The fire drills for the months of July and August were not documented. This was corrected at inspection when the missing drills were documented.

Caregiver/child ratio was reduced to 1 caregiver to 8 infants under 18 months of age during naptime. This was corrected when a second caregiver returned to the classroom and the standard was discussed with the operation.

A caregiver lightly smacked a toddler on the hand as a form of discipline.

A caregiver was observed caring for children without a required background check.

A child was given an identified food they are allergic to.

The caregivers in the playground did not have a list of the ages of the kids in their care.

On 02/28/24 a child was pushed and fell and hit their head and was taken to the ER. The child was taken in for a concussion and was required to follow up with their primary physician.

On March 06, 2024 the school age room had 27 children to one caregiver. The classroom was out of ratio by 1 child.

Three out of six classroom curriculums observed did not have the correct date or was not dated.

A teenager is frequently coming to the operation and having contact with children without having a background check on file under the operation. Corrected at inspection: The person was not present.

Three out of ten children's files reviewed did not have a date of admission.

The changing mat in the infant room was torn and the foam exposed. Corrected at inspection: They changed the mat during the inspection.

Five out of ten children's files did not have any information under the special needs section.

Two out of ten children's files did not have the phone number and address for the doctor/emergency facility under the emergency medical authorization section.
