The two year old classroom was observed having children climbing on shelves and on top of tables, jumping on their cots during the lunch/nap transition time, and when transitioning from the playground back to the building, children made it to the classroom door located around the side of the building while the caregivers were behind the group unable to see the children. Children were attempting to enter the open door or began playing with the infant toys on the porch.

A caregiver was observed while monitoring the classroom camera pull a child by their forearm toward their cot and another child by their lower legs toward the caregiver.

A caregiver was observed yelling at a child during an art project.

Caregivers were prepping at a counter and in a restroom with their backs to the children in the room limiting their ability to see the children in the classroom and hallway.

A caregiver was observed yelling to children in the class to be nice, leave another child alone, to hurry up and get out of a box they were playing in, and telling a crying child "here, bye" after giving them an item they were asking for.

A child showed signs of favoring their arm after getting up from nap and being whiny and wanting to be held earlier. The parent was not contacted until an hour and a half after the child began favoring their arm.

A caregiver was observed grabbing a child from behind by their arms to stand them up suspecting they may have been injured by another child in an attempt to evaluate them for any injuries.

A child was administered medication their parent provided to the operation but wasn't in it's original prescription box.

One of three toilets in the boys restroom was not working. The young playground had gap at the bottom of the fence over 5 1/2 inches and a broken fence board. The larger playground had a gap at the bottom of the fence 6 1/2 inches, a hole in the wall of the building approximately 9 inches, and the grass was in need of cutting.

The large climber on the playground lacked 6 inches of use zone material.

One employee did not have an employee file to be able to review their hire paperwork.

The director and additional driver did not have a a current transportation training certificate on file.

One out of eleven staff training files evaluated did not have training certificates to match the listed trainings completed.

A child with medicaiton on site did not have the original prescription box.

The operation did not have a posted employee list. NOTE: A new list was generated during the inspection.

The director and caregivers who take children swimming away from the operation had not completed water safety training.

Fire extinguishers in the operation were last inspected on 5/2024.

The operation did not have information in their parent handbook regarding the promotion of active play.

There was a broken fence board near the right back corner of the playground with a gap at the ground level measuring 5 1/2 x 7 1/2 inches. There was an additional broken fence board on the playground gate with a gap measuring 7 inches.

The depth of the loose-fill surfacing measured no more than 2 inches under the large climber and rock wall.

A caregiver was present and in direct supervision of children in care without a background check being entered or an eligibility letter on file. NOTE: The caregiver was sent home during the inspection.

There were two windows in the operation that were cracked and spider. One window is accessible to children and if pushed on, can possibly break out of the window frame and cause injury.

The operational policies did not contain a policy regarding children with special needs.

The back fence of the playground is leaning inward toward the play space and needs to be secured. There is a hole in one of the fence boards that could allow a child to become entrapped.

The operational policies did not contain information regarding the promotion of active play for children enrolled in the operation.
