CPR training must be taken in person and include the use of a CPR manikin and both written and hands on skill based instruction, practice and testing.

The provider has not submitted all 30 hours of annual training required as well as the 15 hours needed for the substitute. Inspector has also not received orientation information for orientation for the substitute and a household member. This is a second-recite.

The operation failed to submit the required documentation even after an extension was granted. This is a re-cite. Inspector is still missing orientation information for the primary caregiver, the substitute, and one house hold member. Inspector is also still missing a copy of the operational policies, training hours for the primary caregiver and the substitute, a copy of a photo for the primary caregiver and the substitute, and first aid and cpr certification for the substitute.

Inspector is missing information for two children that the parents reviewed the operational policies. One of the two children's immunizations is still missing and one more child's record is needed for review.

The operation did not have fire drill documentation for 2025. The operation was also missing documentation for dates in 2024 for CO, Alarm detector, and smoke detector. There was no documentation for these items in 2025.

At the time of inspection, children's records were not available for review.

Inspector observed one person present and had unsupervised access to children without a cleared background or FBI fingerprint. At the time of inspection, no initial background check or FBI fingerprint had been submitted for the person in charge.

The operation did not have documentation of four lock down drills or four severe weather drills for 2024, and no drills were documented for 2025.

At the time of inspection, the operation did not have personnel records, orientation, or first aid/cpr certifications for review. There was also no copy of the operational policies was available for review.

There was no Fire Drill documentation available for review for 2023.

Operational Policies were reviewed the following sections were missing: (4) Medical Emergencies. (5) Discipline and guidance policies. (8) Indoor and Outdoor Activity. (9) Procedures for parents to visit the center. (10) Procedures for parents to review the most recent licensing inspection. (11) Procedures for parents how they can contact local Licensing. (13) Procedure on how to conduct health checks.
