It was determined that a childs pull up was not changed for an entire day while in care.

Seven staff are missing the handling, storing, and disposing of hazardous materials training for the last full year. One staff is missing the preventing and responding to emergencies due to food or an allergic reaction training for the last full year.

One child's EpiPen expired and the other medication listed on the Food Allergy Action plan was not available. One child's medication was expired.

According to the information collected throughout the investigation, it was concluded that an infant was given the incorrect bottle due to the bottles not being properly labeled.

During the follow-up inspection, two caregivers did not know the correct number of children they were caring for.
