Four employee files were reviewed. One employee completed the CPR training 100 % online. The training did not include hands on practice.

Four employee files were evaluated. One employee was pending training in topics in emergency preparedness; preventing and controlling the spread of communicable diseases; administering medication; preventing and responding to emergencies due to food or an allergic reaction; understanding building and physical premises; and handling, storing and disposing of hazardous materials.

Four children's files were reviewed. One file did not have the immunization record. Two files were pending updated immunization records.

Four employee files were reviewed. One employee was pending pre-service training in the following topics: developmental stages of children, age-appropriate activities for children, positive guidance and discipline of children, fostering children's self-esteem, supervision and safety practices in the careof children, postiive interaction with children, and preventing and controlling the spread of communicable diseases.

Sixteen month old child was in a group of 15 children. Oldest child in the group was 13 years of age. There was an age gap of more than 18 months between infant child and oldest child in the classroom. Infant teacher was observed supervisng five children. Based on specified age group of 11 months, the maximum number of children in the group with one caregiver could be four.

Director was missing training on the following topics: Preventing and responding to emergencies due to food or an allergic reaction and preventing and responding to emergencies due to food or an allergic reaction.

Sheets in play yards did not fit snugly. Sheets were loose on the mattress.

Directors have not completed required transportation training.

Transportation binder did not have the following information:Mediical authorization, list of children and emergency contacts.

Driniking water was not available during outdoor play.

Two caregivers were missing pre-service training on age appropriate activities, and supervision and safety practices in the care of children. One employee was missing training in the developmental stages of children.

One caregiver was observed supervising 6 children in the infant classroom. Specified age group was 12 17 months. The maximum number of children one caregiver can supervise is 5 children.

The infant daily schedule did not have the opportunity for outdoor play twice a day.

One out of four employees had an expired CPR/First Aid Certificate.

The director was missing the 30 hours of annual training.

A 4 month old infant was observed to be in care with a 23 month old and a 4 year old.

Fire extinguisher, carbon monoxide detector, and smoke detector were last inspected on August 11, 2022.

The Environmental Health Inspection has not been conducted.

Emergency preparedness plan was not available at the time of inspection.

The required Consumer Product Safety Commission form has not been renewed within the last year.

One record evaluated was missing the orientation statement for personnel policies, operational policies, and abuse and neglect.
