Four employee files were reviewed. One file was missing training certificates on allergic reactions, physical premises, and communicable diseases. Second file was missing training certificates on emergency preparedness; preventing and controlling the spread of communicable diseases, including immunizations; administering medication; preventing and responding to emergencies due to food or an allergic reaction; understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and Handling, storing, and disposing of hazardous materials.

Medical treatment authorization forms were not included in transportation binder.

Infant caregiver was observed supervising a group of five children. There was a difference in age of 31 months between the oldest child and the youngest child . There were more than 12 children present at the center.

The root of a tree was observed to be sticking out from the dirt on the outdoor playground, making it a tripping hazard.

Two out of six employee records evaluated did not have the education requirements.

The surfacing in front of the slides was not at the proper depth of six inches.

The fire extinguishers were last serviced on September 2023.

Four out of six employee records evaluated did not have the pre-employment affidavit on file.

One employee did not have an active CPR/First Aid Certificate and was observed supervising children.

The infant room, toddler room, pre-k room, and afterschool room had lesson plans posted that were outdated.

Two employees were missing some of following trainings: Shaken Baby Syndrome/SIDS/Brain Development, Hazardous Materials, Allergic Reactions, Administering Medication, Emergency Preparedness, Building Safety, Communicable Diseases, and Transportation.

Three cribs manufactured on May 2011 were observed to be in use at the operation. NOTE: This was corrected at the time of inspection. Cribs were removed.

Provider did not notify Child Care Regulation of the change in the indoor space. A room was added and children are in care in that room.

An employee was observed to be supervising children without an active, eligible background check.

One out of six employee records evaluated was missing the following trainings: Hazardous materials, allergic reactions, administering medication, and building safety.

One caregiver was supervising 10 children. The youngest child was 13m and the oldest was 36m.

Six children records were evaluated. Two children were pending updated immunization records.

Six employee records were evaluated. One employee completed the CPR training 100 % onilne. Curriculum did not include hands-on skill-based instruction or practice on a CPR manikin.
