While conducting an inspection, It was unclear as to who the person in charge was.

While conducting the walkthrough, There were two classrooms where the caregivers were unsure of the number of children in care.

While conducnting a first aid kit audit. The first aid kit had missing antiseptic solution whiich is a required item.

The surfacing underneath a swing was torn and exposed, a walking beam was also observed overlapping the play space of an area used for swings. Both posing tripping hazards. The walking beams were removed during the inspection by the provider.

Expired supplies were observed in the first aid kit. This was corrected during the inspection when the provider removed the items from the kit.

A caregiver was observed using a personal device and did not see the inspector in the room. This was corrected during the inspection when the caregiver was asked to put the device away.

There is sufficient evidence to support that a caregiver used or has used negative interactions with children in care.

All required director documentation was not submitted.

One staff member is missing CPR/1st Aid training.

Required emergency drills were not being conducted in the time frames required and not being documented. The last monthly fire drill was conducted around October but was not documented. The lock down and severe weather drills are supposed to be conducted four times a year. They were last conducted around October and do not have documentation of being practiced. A fire drill was conducted at inspection.

The last annual sanitation inspection was on 4/5/22. The provider called to schedule one during inspection.

The last documented fire inspection was on 2/11/22. The provider stated there was a fire inspection completed on 4/11/23 but did not have the inspection paperwork available.

The operation had a change in director and did not complete the notification process to licensing.

Out of 6 staff records reviewed, the training certificates were not available for the following: - CPR/1st Aid cards for 3 staff - Administering Medication for 3 staff - Food Allergies for 2 staff - Hazardous Materials for 3 staff - SIDS, Shaken Baby, Early Brain Development for 5 staff The provider stated the trainings are completed but in an on line database that was not able to be viewed during inspection.
