While auditing a first aid kit, the thermometer was not working.

During a file audit. there were 2 children's files missing health statement.

There was an over the counter medication for an infant that did not have directions on the label for an infant. The medication was sent home to the parent.

There was no safety documentation for fire extinguishers, smoke detectors or carbon monoxide detectors being inspected.

There was no documentation of the current fire inspection.

Medication was observed stored in the medication lock box not in the original container.

Sound machines were observed in cribs. Caregivers were asked to remove sound machines from cribs.

There was no documentation of the current sanitation inspection.

The medication authorization from two of the reviewed files were expired.

During the investigation inspection it was found a caregiver did not have their updated CPR certification.

The operation did not report to Licensing within the required 2 days when a child received an injury that required treatment from a medical professional.

The annual fire inspection was past due.

Two caregivers did not have current CPR and First Aid Training.

The annual health inspection was past due.

One child in care did not have required immunizations on file or a health statement.

Out of ten children's records reviewed, one record was missing complete immunizations.

Out of nine personnel records reviewed, the following was observed: -Five records were missing proof of education; -Two records were missing SIDS/Shaken Baby Syndrome training; -Two records were missing background check results; -Two records were missing complete training hours; and, -One record was missing a notarized affidavit of employment.

It was observed that a caregiver did not wash their hands nor the hands of an infant after a diaper change. This was corrected at inspection, as both the caregiver's hands and the infant's hands were washed.
