A sampling of children records were evaluated. One child's record lacked emergency care authorization; one child's record's lacked a health statement; and one child's immunization record was observed incomplete.

This standard was reviewed as part of an investigation and found deficient. One caregiver's CPR training did not consist of consist of a curriculum that incudes use of a CPR manikin and both written and hands-on skill-based instruction, practice, and testing.

This standard was reviewed as part of an investigation and found deficient. It was determined that infant daily reports have not been consistently shared with parents. In addition, it was determined that staff are not consistently completing infant daily reports in their entirety prior to sharing with parents.

The file of one child was missing the following : immunizations: Hep B(1) and PCV (1)

Two out of ten child records evaluated did not have updated immunzation records. The following immunizations were missing- Child 1- MMR (1), Varicella (1); Child 2- PCV(1), Varicella (1)

One caregiver lacked documentation of current pediatric first aid training.

One caregiver lacked documentation of completion of 8 hours of pre-service training prior to being put in a classroom with children.

One caregiver lacked one hour of training to complete last years annual training.

A sampling of children records were evaluated. Two children's records lacked the address of their designated contact. Five children records lacked the names and telephone numbers of persons other than a parent to whom the child may be released. One child's immunization record was observed incomplete. Provided a list of the children and their missing information to the director at inspection.

An employee was present at the operation who had direct access to children without a complete background check. This was corrected at inspection as the individual was sent home.

This standard was reviewed as part of an investigation and found deficient. It was determined a caregiver held a child?s legs down at naptime in an attempt to force the child to sleep at naptime.
