Based on the evidence obtained during the investigation, a caregiver did not share information with incoming caregivers taking responsibility of her group of children. One child sustained an injury, minutes before transitioning to a new classroom with new caregivers. None of the information about sustaining an injury was communicated to the incoming caregivers so the information could not be communicated to the parents received timely information about the injury.
