During the investigation, it was determined that the staff was aware of a child receiving medical attention after sustaining an injury while in care but did not report to Child Care Regulation within two days of becoming notified.

The operation does not have a current Annual Fire Inspection. The lastAnnual Fire Inspection conducted was on 5/17/2024.

1 out of 4 caregiver records reviewed did not have trainings Recognizing and preventing shaken baby syndrome and abusive head trauma, Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS),and Understanding early childhood brain development for annual training year 2024-2025.

It was found during the inspection there was a classroom that had a diaper changing mat that had tears in the material causing the mat to be absorbent. This was corrected during the inspection by replacing the mat with a new mat.

Follow up was conducted and it was found that 2 out of the 3 missing still did not have the hands on skill based training for CPR and has been employed more than 90 days.

1 out of 4 records reviewed was missing the Pre-Employment Affidavit.

It was found that the operation reduced the number of caregivers in a classroom during naptime. This classroom was combined and had an infant in the classroom therefore this classroom was out of ratio. There were 18 children ranging from 17 months to 2 years old. Out of the 18 children in the room there was 1 child under the age of 18 months.

4 out of 10 records reviewed was missing the Pre-Employment Affadavit. 2 out of 10 records reviewed was missing the Notorized Affadavit.

1 out of 10 records reviewed was missing a child's addmission form. 1 out of 10 records reviewed was missing the address for the Emergency Contact person. 1 out of 10 records reviewed was missing Emergency Medical Care information. 1 out of 10 records reveiwed was missing an Immunization Record.

2 out of 10 records reviewed did not have current immunization record on file.

3 out of 10 records reviewed did not have the hands on skill based training for CPR and has been employed more than 90 days.

It was found during the inspection that the last gas inspection was conducted on 6/25/2021.

During the inspection it was observed that the last fire inspection was conducted on 6/16/2021

During the inspection an infant was observed awake in a restrictive device and the caregver reported the child had been in the restrictive device awake for 30 minutes. This was corrected during the inspection when the caregiver removed the child from the restrictive device.

A caregiver was allowed to be alone with a group of children when the conditions of their background check stated that they cannot be alone with a child or group of children. Note- This was corrected during the inspection when a 2nd caregiver without conditions came into the classroom.

The director did not have proof of current CPR/First Aid certification.

One infant's feeding instructions has not been updated since 5/01/2022.

The maintenance checklist is not current as the last one was conducted on 1/03/2022. During the inspection the operations CEO conducted and documented an inspection of the active play space and equipment.

The fire drill is not current as the last one was conducted on 12/21/2021. A fire drill was conducted and documented during the inspection.
