During inspection three of four caregiver files were missing 16 hours of pre-service training.

During the investigation it was determined a caregiver spanked a relative child during operation hours.

During the investigation it was determined a child was able to lock the bathroom door from the inside and an adult was not in the area with the child.

Three children were observed to be in restrictive devices for 30 minutes.

An infant was observed to be sleeping in a restrictive device for 20 minutes before being moved.

Infants were observed being fed snack in their beds instead of in a high chair or table.

During the investigation, it was determined that caregivers are not supervised appropriately and are not directed to comply with standards.

During the investigation, video footage was obtained that shows a caregiver roughly grab a child and pull them to sit down to address the child's behaviors.

During the investigation, video footage was obtained that shows a caregiver sitting in a classroom with children that are climbing, fighting, screaming, and pushing. The caregiver does not intervene or provide activities to engage the children which led to inappropriate discipline and a child being injured.

During the investigation it was found that a caregiver grabbed and pinched a child who was running away causing marks on the skin. The caregiver was dismissed.

During the investigation it was found that the operation had a gas leak. The operation called and requested an inspection from local plumber during in spection.

During the investigation it was found that a parent reported that a caregiver was pinching a child but the operation did not report to licensing.

During the investigation it was found that there are many ceiling tiles in the each room that are stained from the roof leaks and some have mold. There are walls that are water damaged and need repair.

Based on a DFPS investigation, it was determined that caregivers grabbed a child on at least 2 occasions as a form of discipline.

During the investigation it was determined that the caregiver popped a child on the hand as a form of discipline.

During inspection, three pack n plays were observed with very loose fitting sheets. These sheets were removed during inspection.

During inspection, children did not wash hands after playing outside and there is no soap in the restrooms for handwashing.

During inspection it was observed that children did not have outdoor playtime.

During inspection, the following items were not found in the chidlren's records: 746.605(6) Name, address, phone number of emergency contact for all 4 children Child #1 needs the special care statement box checked Child #1, #2, and #4 need the parent handbook (operational policies) boxes checked. Child #4 needs the healthcare Provider Statement or parent check box completed.

During inspection an infant was observed to be in a bouncer for 40 minutes before being removed.

During inspection, four infants did not have required feeding instructions.

During inspection, children under two were able to watch a video on a tablet.

During the inspection an infant was observed to be sleeping in a bouncer seat for more than 8 minutes and was not moved when it was brought to the caregivers attention.

During inspection a 2m old infant and a 5m old infant were asleep in their pack n play covered with a blanket. The 2m old also had 2 empty bottles and 1 full bottle at the foot of the bed.

During inspection three rooms did not have adequate lighting during naptime to see the children.

A fire inspection report was not available at inspection

Individual storage was not available for children's personal belongings.

A health inspection was available.

Interest Centers were not available with toys, games, books, art, etc activities for any of the 10 classrooms.
