There was a bottle of Clorox wipes on a table within reach of children. This was corrected during the inspection when the bottle was moved out of reach.

Several school-age children were sent to use the hallway bathroom on their own. This was corrected during the inspection when the director was informed that children cannot be sent to the bathroom on their own.

A caregiver stated they had thirteen children when they had twelve. This was corrected during the inspection when the caregiver was informed they had twelve children.

The director did not have two hours of transportation training.

An outlet in the toddler room that did not have a cover. This was corrected during the inspection when a caregiver replaced the outlet cover.

The use zones to the sides and at the access point of an indoor slide where not six feet.

The van binder did not have emergency medical treament authorization forms for the children riding the van.

A garbage can was observed to be full of garbage and uncovered on the playground area of the operation.

There was sufficient evidence found during the investigation to show that adequate supervision was not provided during outdoor time.

There was sufficient evidence found during the investigation to show that one child was left unattended on the playground for approximately 1 minute.

Out of four children's records reviewed, one child needed an updated immunization record.

Out of four staff files reviewed, one caregiver was missing training in abuse and neglect, communicable diseases, administering medication, and preventing allergic reactions for the 2023-2024 training year.

The parent handbook was missing hearing and vision screening requirements, criteria used to determine when weather will prohibit outdoor play, a plan for physical activity to occur indoors when weather prohibits outdoor play, procedures for parents to view the minimum standards online, information on employee vaccinations, and procedures for transportation.

During the DFPS investigation, there was sufficient evidence found to show that a caregiver used inappropriate discipline when the caregiver dragged a child by the arm.

Out of six children's files reviewed, one child's file was missing an emergency medical care authorization and food allergy information.

The electrical cord to the small crockpot that is used to warm bottles was exposed to the infants. The refrigerator was moved to cover the cord to the warmer.

Observed tall shelves that were easy to tip over that were not mounted in the school age area. I spoke to the operation about mounting the shelves due to tipping. There was a metal rebar sticking out of the playground near the fence. The rebar was removed from the playground.
