The standard is evaluated for a standard investigation and was found not compliant. Based on the interview with staff, staff observed a young child with marks on their legs, uncleaned clothing, hands, face, crying, and uncombed hair upon arriving at the operation but failed to contact DFPS to express concerns of the young child may be placed at risk away from the operation.

Two children files did not have the address of the emergency contact.

Two children did not immunization records on file.

The driver did not record a list of children that were transported from public school to the operation on September 5th and 6th 2024.

The first aid kit that is stored on a vehicle that is used to transort children did not have a complete first aid kit. The deficiecny was corrected during this inspection when management replenished the items that were missing.

Two infants younger than 2 years were engaged in screen time. The standard was corrected during the inspection when the caregiver closed the screen of the iPad and transition the infants to music time.

This standard was evaluated as part of an investigation by the Department of Family and Protective Services and was found to be compliant. Three children disclosed they were tap on the head as a form of discipline while at the operation.

Infant feeding schedules for infants not ready for table food were not available for review during the inspection.

This standard was evaluated during the inspection and was found to be deficient. Caregivers did not change an infant's clothing after all of the infant's clothing became soaked with milk.

This standard was found to be deficient as inappropriate discipline was conducted after a caregiver admitted to grabbing a child by his shirt while they were on the playground.

One caregiver did not have current training in the prevention, recognition and reporting of child maltreatment.

The current fire inspection was not available for review at the time of the inspection.

The operation allowed a person to be present at the operation before receiving notice from the Centralized Background Check Unit regarding the person's eligibility. The person was immediately removed from the building. Another citation related to background checks within the next 6 months may result in an additional penalty and other enforcement actions.

Sheets are not fitting snuggly on an infant crib mattress.

The following trainings for a caregiver were not available for review at the time of the inspection: Sudden Infant Death Syndrome, Shaken Baby Syndrome and Understanding Early Childhood Brain Development and transportation training.
